Athletic Clearance Process for FHS Coaching Staff

Once your paperwork has been reviewed, the school athletic director will let you know when the
District will call you to set up an appointment.
COMPLETE ACTIVITY SUPERVISOR CLEARANCE CERTIFICATE - ASCC
ASCC clearance is mandated for all coaching staff. Please follow the directions below.
1. If you do not hold an active, valid teaching credential you must apply to the California
Commission on Teacher Credentialing (CTC) for an ASCC (whether you are a volunteer or paid
coach). The ASCC fees are not reimbursed by the District. ASCC fingerprints may be done
prior to obtaining District fingerprints; however, if the District receives a reply from DOJ that
disqualifies you from coaching, the CTC fees are still non refundable. You can make one
appointment to have both sets of fingerprints taken – State required ASCC and District required
fingerprints. If you want to have your District fingerprints and ASCC fingerprints scanned at the
same time please read DISTRICT REQUIRED FINGERPRINTS below.
2. Go to the CTC website: http://www.ctc.ca.gov/. Select the second orange button from the
left called “ONLINE SERVICES FOR EDUCATORS”. Select the big yellow link
“EDUCATOR PAGE”.
3. Enter your Social Security Number and Birth date in the appropriate boxes and click
“OK”. Repeat the process again and click “OK”. Select “Create Person” and enter your
personal information and click “SAVE”. Select “NEW” and enter your address, click “SAVE”
and then select “NEXT”.
4. Scroll down to “Web Applications” section and click “Create New”. In the drop down box
under “General Application Category” select “Certificate of Clearance/Activity Supervisor
Clearance Certificate”. In the drop down box under “Document/Authorization Title” select
“Activity Supervisor Clearance Certificate”. An Activity Supervisor Clearance Certificate
Checklist will display. You may print this Checklist if you wish. Select the link for form 41LS, fill out the form online, print 3 copies and close the popup. Scroll down the page and select
“NEXT”.
5. Read the “Professional Fitness” questions and if you understand the statements select
“YES” and click “NEXT”. Answer the next 10 questions and select “NEXT” to each
one. Certify that all your statements are true and enter the City, County, State, and
Country. Check if you agree and check acknowledge fees are non-refundable. Click “Proceed
To Payment” and wait while a new screen pops up and then enter your payment information.
6. The District and Athletic Director will verify with the CTC website that you have
obtained the required ASCC. The ASCC verification and District hiring procedures are
required prior to any direct coaching.
7. Call Orange County Department of Education “OCDE” at (714) 966-4306 to make an
appointment. There will be a $69.00 (not reimbursed by District) fee due to OCDE. OCDE is
located at 200 Kalmus Drive, Costa Mesa, 92628-9050. Take the ASCC and District forms to
OCDE.
DISTRICT REQUIRED FINGERPRINTS (Livescan)
The Athletic Director will tell you when to call District Personnel at (714) 730-7301 ext.
353 to make an appointment to pick up the District fingerprint packet. Do not come to the
District without an appointment – you will be asked to return to the school to see the
Athletic Director. When you come to your appointment at the District (300 South C Street) to
pick up the forms you will be given directions as to how to have your fingerprints taken through
Orange County Department of Education (OCDE) in Costa Mesa. There will be a $20.00
fee. Save the OCDE fingerprint receipt you are given and bring to your final District
appointment (Orientation) for reimbursement. When the fingerprints have cleared Personnel
will contact you with further directions.
Evidence the following is required:
TB -MANATOUX SKIN TEST If you have not recently (within the last 4 years) had your TB
read, you may ask the District Receptionist for a referral when you pick up fingerprint
information from Personnel. This test will be at the prospective employees’ cost. Bring proof of
TB to Orientation.
CPR CERTIFICATION (ONLINE COURSE NOT ALLOWED) - Bring certification to
Orientation
FIRST AID CERTIFICATION (ONLINE COURSE NOT ALLOWED) - Bring certification
to Orientation
DRIVER’S LICENSE – Bring original to Orientation
SOCIAL SECURITY CARD (Not required for volunteer) - Bring original to Orientation
EMPLOYMENT PACKET (Not required for volunteer) - Complete and bring original to
Orientation
