Consent form: photography, video and PR
Parent/guardian: please complete the following form to indicate whether or
not you consent to your child's images (photographs and videos) being used
for different purposes and whether or not you would be happy to share your
child's experiences. You can ask to see images we have of your child. You
may withdraw your consent at any time by writing to us.

Photography, video and PR: our commitment
We have provided the information below so that you are clear about how your
child's image will be used, what you can do if you want to find out more about
how they are being used, or if you want to complain about the use of your
child's image or story.
Recording images
Photography or filming will only take place with the permission of the
Headteacher and under appropriate supervision. Children will only be named
if there is a particular reason to do so, in line with consent given, and home
addresses will never be given out.
If you wish to attend school events and functions and take photos or videos of
your and other people's children, please be sensitive to other people and try
not to interrupt or disrupt concerts, performances and events. Please also
ensure that you have agreed to appropriate use in section 2 on the consent
form.
Image and story use
We will use your child's image and story only in line with your agreement as
per this consent form. Images that might cause embarrassment or distress
will not be used. If we would like to use your child's image or story in a way
that is not clearly covered by this form, we will contact you to get specific
permission to do this.
Lifespan
We will only use your child's images or story in materials we are producing for
the duration of your child's time at RNIB Three Spires Academy. With printed
materials however it is possible that some of these will be in the public
domain after this lifespan expires. If we would like to use the images or story
in anything that has a significantly longer lifespan, we would ask you for
specific permission. If we are using the photographs on our website, they will
not be used after expiry of this permission.
Images of children for promotional purposes
Wherever possible we encourage parents and carers to talk to their children
about the fact that we are might use these photographs to promote the work
of RNIB and our partner charities and how proud we are of the fact that we

are able to do this. We want to involve children and young people as much as
possible in the decisions we make about how to use these photographs.
Checking how images have been used
The photographs we take will be stored so they can only be accessed by the
teams who will be using them. We will log every use of the images and if you
want to know the details of how they have been used you can contact us at
[email protected] or by calling 024 7636 9500 and asking to talk to our
marketing team.
Withdrawing permission
If you would like us to stop using images or stories then please contact us at
[email protected] or call 024 7636 9500 and ask for our marketing
team.
Concerns
We take all concerns about how photographs have been used very seriously.
If you are unhappy about anything and would like to raise a concern with us
please email [email protected] or call the marketing team on 024 7636
9500. We will investigate your concern and agree a way forward with you to
address your concerns.
