Lead Me Home Regional 10-Year Plan to End Homelessness 2011-2021 WORKING TOGETHER FOR OUR COMMUNITY Background Development of this Plan took approximately 14 months in a series of interviews and meetings with a wide range of stakeholders. Several committees and working groups were formed including the creation of a planning Leadership Council. The Plan call for greater integration of mainstream services and deeper community engagement. The Plan is in alignment with federal and state priorities including those of the United States Interagency Council and positions our counties to be responsive to the Veterans Homelessness 5-year plan. Key Plan Features Creating a unified system/one culture for working across systems (social services, health services, corrections, probation, planning); Making Permanent housing the first priority for all stakeholders. Prevention, Employment, Discharge Planning and Supportive Services. Anchoring all efforts in a network of basic housing assistance and supports across both counties and tapping all resources creatively. Implementation Priority Strategies Provide a full Continuum of Housing Options to help people who are homeless or at- risk to access and maintain permanent housing. Provide services, keep people housed. Support economic stability (employment first, income assistance) Return to Housing (Discharge Planning Efforts medical, correction, and Foster Care ) Next Steps Develop collaborative resources and infrastructure to formalize support for Plan implementation Identify funding strategies to implement plan initiatives Establish outcome measures, track progress Evaluate Success in addressing homelessness Recommendations Recommendations Receive and approve Lead Me Home – The Game Plan for Housing Homeless People in Monterey and San Benito Counties; Authorize the Department of Social and Employment Services and the Coalition of Homeless Services Providers to oversee implementation of this Plan and to establish local planning committees in partnership with stakeholders. Acknowledge the role of the Lead Me Home Leadership Council in pursuing HUD Continuum of Care resources in alignment with the Homeless Emergency Assistance and Rapid Transition to Housing (HEARTH) Act. Conclusion This plan, along with your Board’s acknowledgement of the Lead Me Home Leadership Council launches a local effort and oversight body as recommended by the US Department of Housing and Urban Development to end homelessness and more successfully compete for needed resources The Coalition of Homeless Services Providers and the partners involved in developing the Lead Me Home Plan requested that the Department of Social and Employment Services serve in a leadership role to establish the Lead Me Home Leadership Council, the local planning committees.
