AISD Online Physical Packet and Forms

Go to https://alvinisd.rankonesport.com
2. You can choose from two options:
Option 1: Create a free account, by selecting Register Now, which will allow you to go
back to finish your physical if you have to stop. You will receive a confirmation email.
Option 2: Select No Thanks, and continue onto the Physical Form.
3. AISD Online Physical Packet Home Screen
4. Select +Add a Child
NOTE: If you are adding an incoming 7th grader, “Student information does not
match….” please disregard red message and continue.
5. Select the Download and Print button
6. Select the Physical & Medical History Form
7. Click on the Student’s Name blank and begin filling out the form. You can click or tab to the
next entry.
REMINDERS…
When entering the Date of Birth, please enter it as mm/dd/yyyy format
When entering a Phone number please enter it as xxx-xxx-xxxx format.
8. Preparticipation Physical Evaluation – Medical History form
This information cannot
be filled electronically,
it must be printed and
signed in person.
This will be completed by
the Athletic Trainers.

 Completely fill out the information
 Use your computer mouse to sign the student and parent/guardian signature.
 After completion and signatures, select Submit.
