Dear 5th Grade Families,
Miss De La Fe- ELA/Social Studies
My name is Taylor De La Fe and I am thrilled to be
focusing on Reading and Social Studies curriculum this year with the
students. These are my two most passionate areas of study, and I cannot wait to
see what these kids can do! A little about me… I am originally from California, but
moved to Arizona in 5th grade, so I consider the beautiful state of Arizona my
home. I graduated from Arizona State University in with a Bachelor’s of Arts in
Elementary Education (GO DEVILS!). I am engaged to my high school sweetheart,
Austin, who has always supported me throughout all of my adventures. We enjoy
spending time with friends and family, traveling, and are currently preparing for our
wedding this September. (YAY!) I cannot wait to meet you all, and look forward to a
great school year!
Mr. Cortez- Math/Science
My name is Daniel Cortez and I am excited to be teaching Math and Cottage to
your kids this year! I graduated from ASU with a Bachelor’s of Arts in Education,
focusing on STEM (Science, Technology, Engineering, and Math). I was born and
raised in Mesa, Arizona, where I attended Catholic grade school and Catholic high
school. I come from a family of musicians, so I play in a band with my parents and two
younger brothers in my free time. I’m an avid Star Wars fan, Lord of the Rings fan,
super hero fan, and Harry Potter fan. I look forward to a great year with your
children, and getting to know you all!

Miss De La Fe and Mr. Cortez’s 2017-2018 Supply List
Because students will be switching classes this year, we want to make sure their load is as light as possible.
With that said, there will be some items they will need to carry back and forth with them that will be designated
as theirs and others they will leave in one class or another to be used by everyone. Please keep that in mind
when purchasing supplies. While we know some students will want fancier pens and such, we need to be sure all
students are accommodated for, so sticking to the more generic supplies is best. This list contains everything
needed for both classes- only purchase things once!
• Please print off the Open House forms, fill them out, and bring them to turn in when we meet on July 24th.
• Purchase the Student Planner ($6) and Scholastic News subscription ($6) at Open House. Checks made
payable to EDUPRIZE PTN.
The following Items will be for personal use and should be labeled with your child’s name (colors are
specified for different subjects):
(1) 21/2 inch binder with clear view front
(1) pencil pouch
(1) set of headphones (for chrome book use)
(3) 3 prong folders red, green, and yellow
(1) 2 pocket folder **for science specials classes- does not need prongs
(1) composition notebook (college ruled)
(1) spiral notebook (college ruled) red
(6-12) pencils
(4) pens (blue and/or red)
(1) scissors
(1-2) large pink erasers
(2) sharpies (black)
(4) white board markers (thin, black)
(1) pack colored pencils or crayons
(1) pack markers
(1) ruler
The following items will be for class use and should not be labeled:
(1) ream lined loose leaf paper (college ruled)
(4) glue sticks
(1-2) packages sticky notes
(1) pack 100 index cards- to be used for Wax Museum project
(3-5) clear sheet protectors
(4) highlighters (preferably 2 multi colored)
(1 or more) box of tissues
(2 rolls) paper towels
(2) reams copy paper
(1) 1/2 inch binder with clear view front (to be used as reading portfolio)
To ensure enough supplies for everyone, these items have been split amongst boys and girls.
Boys: Clorox wipes, hand sanitizer, gallon size ziplock bags
Girls: Baby wipes, hand soap, quart size ziplock bags
The following items are wishlist items. Do not feel obligated to purchase any, but all donations are
greatly appreciated. In addition, we would greatly appreciate any duplicate items from the required
list.
