2． Request
(1) Request
The player requests the intermediary to negotiate with clubs to conclude a player contract for the player.

3． Remuneration
The player will pay the following remuneration to the intermediary.

4． Termination of the contract
The player or the intermediary may terminate this contract with immediate effect by notifying to its
counterparty (the intermediary or the player) in writing, provided, however, the party who terminate the
contract shall compensate for damage to its counterparty.
5． Other agreed items
In accordance with JFA’s Regulations on Intermediaries, copies of this contract and other agreements for
supplementing this contract shall be submitted to JFA.
6． Compulsory provision
The parties agree that they will comply with the statutes, regulations, directives, and decisions of FIFA,
JFA, confederations, and associations, and the laws and ordinances of countries, including Japan.
7． Dispute settlement
(1) Should a dispute arise between the player and the intermediary over the interpretation of this contract
or the execution of this contract, the player and the intermediary will settle the dispute through mutual
consultations in good faith.
(2) If the dispute is not settled thirty (30) days after requesting the consultations in the preceding
paragraph, the player or the intermediary may ask the JFA’s Consultative and Mediatory Committee to
render conciliation and mediation services pursuant to the provisions of JFA’s Statutes.
Two copies of this contract have been signed, with the intermediary and the player each retaining one copy.
