LV Female to Play U15 or U17 Boys Competition

PERMIT FOR A FEMALE PLAYER TO PLAY IN U15 OR U17 BOYS COMPETITION
This form must be completed by a junior’s parent/guardian and the Club Personnel listed below if a female
player wishes to compete in an Under 15 or Under 17 Boys game. Girls are not permitted to play in Under 15
or Under 17 boys unless this form is completed and received by Lacrosse Victoria prior to the participation in a
game. A permit will only be approved by LV if all designated personnel have signed the form.
By signing this form, the parent/guardian and club is accepting the responsibility for any risk involved in the
female player competing in an Under 15 or Under 17 Boys Game. The permit is valid for ONE year ONLY.
Lacrosse Victoria may at any time during the season revoke a permit should it receive advice of any concerns
about the player’s capacity to safely continue to participate in either Under 15 or Under 17 boys. By signing
this permit form you acknowledge and agree to the following statement.
We understand that lacrosse is a contact sport (stick and/or body) and by allowing the named individual on
this form to play in an Under 15 or Under 17 Boys competition, she will be competing against males in a
male competition. We understand and accept that there may be a significant increase in the level of physical
contact and that the player may be exposed to adult behaviours.
By signing this form you are agreeing for the below named player to play in the Under 15 or Under 17 boys
competition for the year of the application.
