Shaping Our Future -- Strategy Hive #2
“Reorganizing care and services according to individual needs and life goals”
Tuesday February 7, 2017
4:00pm – 8:00pm, Holland Bloorview Cafeteria
Introduction:
Thank you for agreeing to participate in our strategy planning process. We are looking forward
to a rich discussion that will inform our vision and strategy for the future. This strategy hive will
focus on exploring novel organizing principles for care and services that are centred around
client and family needs and life goals, rather than structured based on diagnosis.
Preparation for the session:
Please review this briefing note, and think about and make notes on these questions:
• What opportunities would a focus on needs rather than diagnoses bring to care and
services for kids and families at Holland Bloorview?
• What meaningful models or concepts could we use to ensure that we are factoring in
all of the holistic needs of children and families as we design and provide care and
services?
• Where might we need to round out our care and services to better meet client and
family needs and life goals, including mental health supports?
Background:
As part of this strategy hive, we want to have a dialogue about what would happen if we
focused attention on meeting needs and goals centred around models like the 6Fs (function,
fitness, fun, family, friends, future) and whether there would be more meaningful ways to
frame and offer our services to better meet the needs of families at each stage of their journey.
With these lenses in mind, where might we need to round out our service offering and where
might we have opportunity to restructure services to optimize care, outcomes and experience?
And, in tandem, how can we bring an academic lens to this conversation to explore
opportunities for research and teaching in this domain?
1
Framing questions for the session:
Where is diagnosis most useful in supporting clients and families in meeting their goals
and needs? Where is it least useful?
What unique and novel frames could we bring to how we design and offer services to
better advance the needs of families at each stage of their journey and to meet
individualized goals?
Where might we need to round out our service offering? Where might we have the
opportunity to restructure services to optimize care, outcomes and experience and to
generate better outcomes across all life domains (function, fitness, fun, family, friends,
future)?
How could these new frames and models improve the way we assess quality and safety?

Rationale for this topic (based on pollination sessions with families, kids and staff):
•
•
•
Through many of the pollination sessions, people expressed a desire to have all
services available to all clients regardless of diagnosis, with a focus on a wellness
model versus a disability model.
Families want a holistic approach to treatment, which includes a team that meets to
advocate for and to address the needs of the whole child and continually connects to
review and modify goals.
Many expressed a high value on a “6Fs” (function, fitness, fun, family, friends, future)
approach to care and goal setting that keeps a balance of all of the F’s in the design of
care, services and experiences (including access to more social time to create
valuable friendships).
Illustrative quotes from kids, staff and families:
•
•
•
•
•
“Look beyond diagnosis to the functional needs of each child”
“Right now diagnosis is important for access to the system and service delivery but
what happens after diagnosis should be needs based”
“Every child and family should have care and experiences that reflect their individual
and unique ‘6F’ needs and possibilities (Fitness, Function, Friends, Family, Fun and
Future)”
“2 kids with same need but different diagnosis get vastly different services – need
equity of service. “
“We need to reorient our purpose- it’s for children to become happy and healthy
adults.”
