Risk Warning:

You should be aware that there are risks of injury with playing basketball, as there are with most sports. Risks will arise in the context of the
activities of running, catching, throwing, shooting and guarding opposing players. While we aim to minimise risks, it is not possible to
eliminate them all.
Privacy Statement:

MDBC, SDBA and NSW Basketball Association Ltd (“BNSW”) collect your personal information to assist in providing products and services you
have requested. If you do not provide this information we may not be able to register you. You can gain access to your personal information by
contacting SDBA on 95421999 and BNSW on 97462969.
Player and Team Obligations:









Players are expected to attend ALL games throughout the competition.
Players must pay the required game fee to the front desk, whether or not they attend games.
When a player is unable to attend a game, advance notice is to be given to the Team Manager and Coach; at least one week or as soon as
possible.
Players are to be financial members of the club and correctly registered before they are permitted to play in the competition.
All teams can avoid a forfeit situation by using players from within the club, i.e. Players from a lower grade or age group.
When a team forfeits a game they receive no points for that game and are obligated to pay game fees. Forfeit fines may be
applied for senior games.
All players must be correctly attired in team tops.
During a game the COACH is entitled to approach the referees if he/she is dissatisfied with the score on the board or the score sheet.
Each team is required to provide a scorer for the bench.

I shall strive to be a team player, and display a true sportsmanship attitude towards other players;


I appreciate that important family commitments can take precedence over sport;
As a player I will inform my coach of my commitments to other sporting bodies and accept any decision made by the coach based on my
stated availability to play;
My participation in any particular game will be at the discretion of the coach and will be for the benefit of the team as a whole, not
individuals;
I will report any activity or problem, which may harm players or the reputation of the Club to team officials as soon as possible;
I shall conduct myself in a proper manner;
I understand that inappropriate behaviour or attitude detrimental to the team or the Club may result in disciplinary action or
expulsion from the competition; and





I agree to respect Club facilities and abide by the code of conduct set down for the use of those facilities.
Filming and Photographing of Players:




The club is happy to permit the videoing of games and photographing of players by parents and relatives of players from time to time.
The videoing of games and photographing of children by persons other than parents and relatives is strictly prohibited.
Where people not related to players are found to breach this rule they will be asked to stop. If the request is not followed then the party will
be reported to centre management and asked to leave the premises.
From time to time the club may photograph or video record games for inclusion on its web site or as part of its marketing activities.
Permission from both competing teams and players will be obtained prior to this taking place.

I wish to apply for/renew membership of MDBC and SDBA and agree to abide by their Rules, By-laws and Code of Conduct. I also acknowledge
that if I participate in any competitions conducted under the auspices of BNSW, I will be bound by the Tribunal By-laws as adopted by and varied
from time to time by BNSW. I further acknowledge that I have been afforded the opportunity to inspect said By-laws at the time of signing this
form.
By joining MDBC and SDBA I then become a registered participant of NSW Basketball Association Ltd.
