In exchange for participation in the activities of baseball training, clinics, practices and games organized and administered by the teachers, coaches, students
or agents of Woodcreek High School of 2551 Woodcreek Oaks Blvd, Roseville California 95747, and/or the use of the property, facilities and services of
Woodcreek High School, I agree for myself and for the members of my family to the following:
1)
I agree to observe and obey all posted rules and warnings, and further agree to follow all oral instructions or directions given by teachers,
coaches, students or agents Woodcreek High School.
2)
I recognize that there are certain inherent risks associated with the above described baseball related sports activities and I assume full
responsibility for personal injury to myself and family members, and further release and discharge Woodcreek High School and any teachers,
coaches, students or agents affiliated with Woodcreek High School for injury, loss or damages arising out of my or my family’s participation in
these activities.
3)
I agree to indemnify and defend Woodcreek High School and the teachers, coaches, students or agents of Woodcreek High School against all
claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise
from my or my family’s participation in the above noted activities.
4)
I consent to the participation of my minor son/daughter named below in the activities of baseball training, clinics, practices and games, and agree
on behalf of my minor son/daughter to all of the terms and conditions of this agreement. By signing this Release of Liability, I represent that I
