Electronic Endorsement Application Form
The 1st4sport Endorsement Service aims to identify sector-relevant educational solutions which make
people better prepared to fulfil their role in the industry. To have a solution assessed to become part
of the service the developer/owner needs to submit to 1st4sport:
A a completed Endorsement Application Form.
B a cheque/Purchase Order for the non-returnable Initial Endorsement Assessment Fee (£350 +
VAT).
C copies of existing materials in support of the Application Form which demonstrate how the
educational solution meets the endorsement criteria.

application and assessment process should be addressed to:

Please complete this form on a computer and either print and attach to hard copies of documents in
support of the application; or email to the above address. An alternative PDF form is also available to
write on. Please allow 20 working days for the assessment to be completed and a response to be
issued.
