This page will help you to understand what all of this information means. A. Key Line - This section displays: Voter ID – this is the unique number assigned by SCORE to each voter record Last Name, First Name, MI – Voter’s last name, first name, and middle initial or name Suffix – Jr., Sr., II, III, IV Birth Date, Age, Gender, Driver’s License # and the last four of the voter’s SSN (if provided) B. Residence - This section displays: The voter’s Residence Address, Mailing Address and Ballot Address - as well as their Precinct and Split (if the address is valid) Current County C. Registration - This section displays: The voter’s Party affiliation and the Date they affiliated with their current party The Transaction Source is the source of the most currently received document The voter’s Status and Status Reason The Move Date is the day the voter moved to a new residential address The County Reg Date is the day the voter registered with the county The Effective Date of the voter registration application or change of registration information that affects voter eligibility (residence address, affiliation or status) D. Document Image - This section displays the voter’s scanned images and captured signature 3|Page Revised 3/16/2015 Registering a New Voter At this point you have performed the following: Conducted a voter search and was unable to find an existing voter record Registering a New Voter 1. Click Voter Registration from the main SCORE screen 2. Select New in the upper left corner Key Line 3. Enter the voter’s information on the Key Line starting with Last Name (as seen below). 4. VERY IMPORTANT: Required to use the TAB key through all key line fields so the system can initiate the verification check through all Colorado agencies and duplicate checks. The TAB key should also be used as you enter information throughout the entire screen. 4|Page Revised 3/16/2015 Registering a New Voter 5. Pay close attention to the message that appears after SCORE performs a systems check through all Colorado agencies. Choose from one of the following messages: IF THEN The message in the upper left corner reads: System has performed CDOR, CDPHE, CDOC, and duplicate checks on the entered data Proceed to Step 6 The Duplicate Voter screen appears and the voter is already registered Highlight the voter record that meets minimum matching criteria and click SELECT Select RE-REGISTER if the voter record is Cancelled IF you receive a message indicating the voter record is Cancelled and it is the only existing record for that voter, click RE-REGISTER and the OK button to confirm the voter has submitted an updated registration. Proceed to Step 6 The Duplicate Voter screen appears and there is not a voter match Select NO MATCH and proceed to Step 6 The voter has multiple voter records Refer to the Addendum A on page 9 The CDOR, CDOC or CDPHE Verification screen appears Refer to the Addendum B on page 10 Residence tab 6. Click the Voter Initiated radio button 7. Enter the voter’s Residence Address: A. House # B. Pre-Direction (if applicable) C. Street Name, D. Street Type, and E. Unit Type and Number (if applicable) TIP: City and Zip Code auto -populate if you place the cursor in the Post Dir field and tab through to the County field SCORE does not accept the address: Research the Address Library and other resource tools (i.e. Assessor’s website, USPS, etc) 5|Page Revised 3/16/2015 Registering a New Voter 8. Enter the voter’s previous address if provided by clicking on the Prev Registration tab. 9. Enter the Mailing Address, if a mailing address was provided and it’s different than the residential address 10. Enter the Ballot Address, if the voter provided a third address where the ballot is to be mailed. This field is for both domestic and UOCAVA international addresses Registration tab 11. Enter Party and verify the Affiliation Date is Today’s Date. [If received by mail, use the Postmark or Received Date] 12. Use the drop-down arrow to select the applicable Transaction Source 13. Ensure the Status is A – Active [Status Reason will default to 2DP - 20 Day Period for a new registration] 14. Select the US Citizen checkbox 15. Enter the Move Date [More details on page 12, Addendum C] 16. Verify the County Reg Date and the Effective Date are Today’s Date. [If received by mail, use the Postmark or Received Date .If received from CDOR or OLVR, use the Submitted Date] 17. Click the Contacts tab and enter the phone number and/or email address, if provided Important Note: If the voter requested email notifications: Checkmark the Receive Email Communication box 6|Page Revised 3/16/2015 Registering a New Voter Absentee tab – UOCAVA Voters Only UOCAVA voters must be flagged, if you haven’t already done so 18. Click the Absentee tab 19. Place a checkmark in the UOCAVA box 20. Select the UOCAVA Type 21. Absentee requests are only created for UOCAVA voters who request their ballot sent by a method other than mail. Choose from one of the following scenarios: IF THEN The UOCAVA voter had a ballot address The ballot address should have been entered in the Ballot field. Do not setup an absentee request. Skip to Step 24. The UOCAVA voter requests their ballot sent by a method other than mail Proceed to the next step to setup a Date Range Absentee request 22. Click the Add button 23. Setup a PMIV Date Range Request A. Select Date Range Absentee for Absentee Type B. Enter the applicable date in the From field C. Place a checkmark in the PMIV box [The To fields auto fills 150 years in the future] D. Select the applicable Absentee Application Source E. Select the applicable Issue Method Note: SCORE properly checkmarks the Residential or Mailing Address box. Do not uncheck or change the default. F. Choose ONE per the voter’s request: Enter an email address or fax#. G. Select the button applicable to the situation: Print Now or Print Later 7|Page Revised 3/16/2015 Registering a New Voter Save the Voter Record 24. Click the Save button at the top of the screen and Active Ballot will appear below the Residential section (during the election cycle) 25. Write the Voter ID# displayed in GREEN in the upper left corner of the SCORE screen on the registration document (in the upper right corner) 26. If issuing a mail ballot, refer to the Issuing and Replacing a Mail Ballot Instructions OR VSPC Instructions, depending on the situation. If issuing an In-person DRE Ballot or an In-person Paper Ballot, refer to the VSPC Instructions. Did the voter’s Last name change? If the last name changed, it is still a voter match provided all other minimum matching criteria exists o Verify Date of Birth, DL and SSN numbers were entered accurately. If the DL was entered correctly and it is being used by another voter, go into each voter record to view scanned images or CDOR Search to resolve the discrepancy o Correct typos on the Key Line, if applicable. Then tab beyond the SSN field again to initiate another SCORE check o Go back to page 5 if the issue has been resolved If the issue has not been resolved, conduct further research. You will not be able to save the record until you have resolved the issue CDPHE – Colorado Department of Health The CDPHE screen appears and the record is not a match The CDPHE screen appears and the record is a match o Click NO MATCH o Go back to page 5 to continue processing the voter o Research the validity of the cancellation o An individual who was cancelled because of a CDPHE match that he/she believes was erroneous has 2 options: The person can go in person, present ID and explain that the match was an error; or the person can send a signed statement that includes their name, residential address and date of birth and that explains the error. If the elector does either, he or she is reinstated. 10 | P a g e R e v i s e d 3 / 1 6 / 2 0 1 5 Registering a New Voter CDOC – Colorado Department of Corrections The CDOC screen appears and the record is not a match o Click NO MATCH o Go back to page 5 to continue processing the voter The CDOC screen appears and the record is a match and the voter states s/he is on probation The voter is eligible to vote The CDOC screen appears and the record is a match and the voter states s/he is on parole The voter is not eligible to vote. The CDOC screen appears and the voter claims that s/he: Was cancelled in error Is not currently serving a sentence or confinement for a felony conviction Is not serving a sentence of parole for a felony conviction 11 | P a g e R e v i s e d 3 / 1 6 / 2 0 1 5 o Go back to page 5 and continue processing the voter o Cancel the voter’s record o Do not issue a regular ballot; however you can issue a provisional ballot if the voter still insists on casting a ballot o Have the voter fill out the Reinstatement Form – CDOC Match (Form #180) on Clerks’ Corner under SOS Approved Forms. Registering a New Voter ADDENDUM C How to use Voter Registration’s Move Date Field The Move Date field is now available in Voter Registration. Enter the date provided by the voter. Not all voters provide complete Move Date information. In this circumstance, enter a Move Date following the guidance below. If the voter does not supply a Move Date and is a new record in the state: o Enter the Effective Date in the Move Date field. If the voter supplies a Move Date that is only a year: o Enter the 1st day of the month you are keying in the document and the voter supplied year. Example: Voter writes “2012” and you’re keying it on March 17, 2014. Enter 03/01/2012 in the Move Date field. If the voter supplies a Move Date that is only a month and year: o Enter the 1st day of the month provided and the year supplied by the voter. Example: Voter writes “June 2013” and you’re keying it on March 17, 2014. Enter 06/01/2013 in the Move Date field. If the voter does not supply a Move Date and a change is being made to their residence address: o Enter the Effective Date in the Move Date field. If the voter is updating their voter record, but does not change their residence address and does not supply a Move Date: o If there is an existing date in the Move Date field, do not change the date. o If the field is blank, do not enter a date in the Move Date field. If the voter is updating their voter record, does not change their residence address, but does supply a Move Date: o If there is an existing date in the Move Date field, do not change the date. o If the field is blank, enter the supplied move date in the Move Date field.
