NEW PLAYER REGISTRATION INFORMATION 2017-2018
New players, players moving to Vernon, or players from adjacent
Associations. Please fill out New Player Registration form and submit with
copy of Birth Certificate and Health Card.
Please read the following before filling out your registration:

GVMHA offers a beginner hockey program for 4 year olds.
Player registration will be accepted from now through July 31st ~ payment must accompany
registration. After July 31st you may be placed on a wait list (Kidsport or payment plan available
by application at the office).
• Players wanting to try-out for a rep team will pay a $100 tryout fee. This fee is included in the
registration fee schedule under Rep section, and covers ice time, evaluators and officials.
• All players who indicate they want to try-out for Rep will be contacted with their tryout times via
email. GVMHA must be notified 7 days before tryouts begin if you decide not to try-out.
• Carding/Team Fees - Players selected for Rep teams only will pay an additional ice/carding fee
once teams are finalized ($250 for Atom Dev to MD Tier 3 players, and $360 for Midget Tier 2
players). Players on Recreational teams will pay an additional fee for the Superleague Playoff
tournament (approx. $40 each). Other team fees will include tournament fees, team parties etc.
Rec team fees are approximately $150 - $250 per season ~ depending on fundraising and how
many tournaments you attend.
• If there are more than 2 players in a family, the 3rd and 4th players will receive ½ price on their
registration fees (no discounts on rep tryout fees).
• Female players - please register with a paper registration if you would like to play female
only. Team structure will depend on the numbers registered. Females wanting to play in both
female and male divisions will pay an extra $250 registration fee. A GVMHA all-female Bantam A
Rep team is planned for the 2017-18 season. The Bantam/Midget Female team will play out of
Lumby and the Midget A Female team will play out of Salmon Arm. Please register in kind with
GVMHA first before registering with those Associations.
• The third section of the New Player registration form is for the parent/guardian information. Please
ensure that one email address is included on the registration form, as GVMHA will be
distributing information by this means. 1 email per player will be used by GVMHA.
• GVMHA will assist in seeking financial assistance in conjunction with Kidsport and a payment plan
application is available at the GVMHA office.
• Pro-rated refunds are available up to December 31st, minus a $40 administration fee.
• No refunds shall be given to suspended players.
• There will be a $35.00 charge for NSF cheques.
ALL players wishing to try-out for the Regional Major Midget Team (Kelowna) MUST register with
GVMHA first before trying out (no payment will be taken unless player is back in Vernon).
PLAYERS WHO ARE MOVING TO VERNON:
If moving from another Minor Hockey Association, the Move with Parent form, along with a proof of address
(tenancy agreement, utility bill, driver’s license with new address) must be filled out and received by GVMHA before
the player is permitted to play. (Forms are available on our web site or at the GVMHA Office). A copy of the new
player Birth Certificate & Care Card is also required.
Player registration, form of payment (Visa/MC,cheque or cash), Player Movement Form (if applicable) and a copy
of players Birth Certificate and Care Card, must be signed and mailed to the following address or handed in at the
