We will observe a moment of silence at 11:00am to commemorate the workers who have lost their lives.
Guest speakers.
After the speeches, there will be an open forum for anyone else who wishes to share their story.
Ball Hockey:
• 40 teams max; first come, first serve for registration (see attached registration form);
• 3-on-3 tournament (3 players, 1 sub, 1 goalie =5 total);
• If we receive enough interest, we will have competitive and non-competitive divisions;
• $25 per player registration fee;
• The tournament schedule will be emailed to everyone April 26th, 2017.
Volunteers:
• Volunteers are greatly appreciated. We are looking for referees, score keepers, raffle attendants and cooks
(see attached registration form).
Donations:
• Donations of food, cash and prizes in any amount are greatly appreciated – food and prizes going to the
event (see attached donation form);
• Donations can be from a person or business;
• Deadline for food and prize donations is Wednesday April 26th; cash donations will be accepted until the
end of day April 29th.
Food and Beverages:
• There will be hot dogs, hamburgers, chips, pop, candy etc.;
• Purchasing of food and beverages will be done by donation.
**All cash donations received will go to Threads of Life**

The deadline for registration is April 17th, 2017. Registration fee is $25 per player and is due on game day. If
you wish to enter more than one team, please complete a separate form for each team. Maximum of 40

Tournament Structure:
This will be a modified double knock-out style tournament. Everyone is guaranteed at least 2 games, with an
opportunity to play up to 8 if you make it to the finals. There will be 3 divisions with a maximum of 16 teams per
division. The top team from each division will play for the championship.
