Student Work Permit Document

REYNOLDSBURG CITY SCHOOLS
WORK PERMIT APPLICATION
STUDENT MUST LIVE WITHIN THE REYNOLDSBURG CITY SCHOOL DISTRICT.
Ohio law states that all employed minors between 14 and 18 years of age must have a
work permit. A new work permit is required each time the student changes place of
employment. Once a student turns 18, he/she is no longer required to have a work
permit even though he/she is still a student. Work permits are issued through the Ohio
Department of Commerce and their system will not enable students who turn 18 to be
entered into their system.
PLEASE FOLLOW THESE STEPS:
1. COMPLETE APPLICATION (BOTH SIDES) BEFORE RETURNING TO:

2. STUDENT must bring in application PERSONALLY so he/she can sign the
permit that is issued from the Ohio Department of Commerce website.
3. PROOF OF AGE: must provide birth certificate, hospital certificate, baptismal
record, or passport. One proof is required. We CANNOT accept a driver’s
license, social security card, or state I.D. as proof of age.
4. ALL 3 SECTIONS MUST BE COMPLETED BEFORE TURNING IN
APPLICATION:
a. Page 1 - Student information (including parent signature)
b. Page 1 - Employer information (including employer’s Tax ID number)
c. Page 2 - Current physical is required. Physicals expire one (1) year after the
date of student’s doctor appointment.
