Honor & Memorial Bricks
The Welcome Square at Baylor Irving is a display of
engraved bricks that welcomes tens of thousands of
visitors each year at the hospitals main entrance. The
bricks commemorate births, anniversaries and other life
events and achievements – making each a beautiful and
meaningful work of art creating a legacy.
Brick Ordering
Step 1 – Fill out the appropriate order form in this packet for which size brick (4” x 8” or 12” x 12”).
Step 2 – Return the form and pay for the bricks at the Irving Healthcare Foundation inside Baylor Medical
Center at Irving located on the first floor at 1901 N. MacArthur Blvd., Irving, TX 75061

Step 3 – You will receive an email to verify the proof of your brick(s) and a letter confirming your order and tax
deductible donation for IRS tax purposes.
Step 4 – After confirmation of your brick(s) proof you will be notified once your brick has been installed.
Please note that bricks are installed two to three times each year.
