Please print as clearly as possible! All information has to be entered into the computer.
Please ensure the following is completed at start of Game:

Even though there is only a space for the Timekeeper and Scorekeeper signature, please clearly print
your name above the boxes requiring your signature.
Also please remind the referees to CLEARLY print their name in the box that says Name (please print)

Goals – please do not forget to fill in the last column indicating type of goal.

 Player #6 is assessed a 4 minute Hit to the Head penalty at 14:46 in the 2nd Period, this is
how it should be completed if no goal is scored during penalty

DO NOT AT ANYTIME MAKE CHANGES TO WHAT THE REFEREES HAVE TOLD YOU
TO RECORD ON THE GAMESHEET. The referees may make an error on the correct player that
scored the goal or the ones receiving assists. It happens; just remember that Changes cannot be made to
the official game sheet.
Penalty Codes to Use:
BENCH – Bench Minor
BOARD – Boarding
BUTT – Butt ending
CHARGE – Charging
CCK – Cross Checking
CFB – Checking From Behind
CTH – Check to the Head
DELAY – Delay of Game
ELBOW – Elbowing
FIGHT – Fighting
GINT – Goaltender Interference
GMISC – Game Misconduct
GROSS – Gross Misconduct
HISTK – High Stick
HOLD – Holding
HDPK – Holding the Puck
HDSTK- Holding the Stick
HOOK - Hooking
INT - Interference
KNEE - Kneeing
MATCH – Match Penalty
MISC- Misconduct (used for 10 Min. Misc)
PSHOT- Penalty Shot
ROUGH - Roughing
RAWSLASH - Slashing
SPEAR - Spearing
TMM – Too Many Men

USC- Unsportsmanlike Conduct
For Goaltender’s Record:
Enter the following information in the Goaltender’s Record section for the Goaltender who
started the game in the top row for Actual Shots. If the backup goalie goes in net, please enter his stats
in the second row.
Goaltender’s Minutes Played: is for the starting goalies only. So if they play the entire game the
columns should be 20, 20 and 20. If the backup goaltender comes in say in the 3rd period at the 14
minute mark, the numbers entered on the score sheet should read 20, 20 and 6.
If a goaltending change is made please enter the new shots and time played on a separate line and
make a note on the bottom of the game sheet at which point of the game and the team that made the
goaltending change.
Shortening of the Game Clock
Please note at what time the game clock is dropped to 2 minutes when this occurs.
End of Game:
 Complete Time Game Ended (under signatures)
 Write total Goals in big number in each section and draw line through empty spaces
 Draw diagonal line through empty penalty spaces to indicate those were not used
 Sign game sheet, have timekeeper sign and referees. Referees will take the sheet if they
need to complete an incident report.
 Return to team manager unless the Referees take the book for incident reporting.
