We will require you to sign a contract with your supervisor: The following information is designed to help you to understand the policies, procedures, and responsibilities of this traineeship/internship position. Please keep a copy for your reference so that there are no misunderstandings between us regarding policies, procedures, and responsibilities involved in this traineeship/internship position. I am a licensed Marriage and Family Therapist. I was licensed by the BBS on this date: 01/31/2005. I have sufficient experience and training to supervise. I have completed the appropriate coursework to supervise both marriage and family therapy trainees/interns and clinical social work associates, and continue to take all required continuing education relevant to my supervisory status. My license is current and in good standing with the board. All supervision you receive will be in accordance with in Title 16, California Code of Regulations, Sections 1833 & 1833.1. Your internship will include 1 hour of individual supervision per week, and 2.5 hours of group supervision including training allowing you to see an average of up to 10 clients in traineeship or more clients per week if you have a current BBS Intern Number. If for any reason your caseload exceeds the number of clients, I am willing to provide the extra supervision necessary in order to assure all the hours that you obtain will count toward licensure. If you cannot attend supervision one week at our regular designated time, I will make a reasonable attempt to offer you an alternative time. If no alternative time can be worked out, or no advance notice is given, then any hours accrued in that week will not count. If you have been hired “at will” (as is allowed in California), you have been hired by mutual agreement, and your internship can be terminated by either party “at will.” If there is any reason for me to terminate your traineeship/internship, you will be given 1 (one) week’s notice and feedback about your performance. In such a case, I will sign for all actual hours performed by you, through the end of your final week of volunteer training as my supervisee. It is your responsibility to maintain copies of your hour’s sheets and provide them regularly for supervision. We prefer to sign hour’s sheets in weekly individual supervision. In order to ensure accuracy, please do not bring multiple weeks to be signed at one time. When working with your clients/patients, you are required to identify yourself as a trainee and/or intern under my supervision. If you have difficulty explaining your role as a trainee or intern, notify me so that we can work this out together as a supervisory issue. I will monitor your work through weekly supervision of your cases, progress notes, and occasional audio of sessions. I may even sit in on a session in order to understand a case more fully. I will give you advance notice when these procedures are planned, and your client will always have the final say as to whether or not s/he agrees to be observed. Most of my interns have found direct supervision rewarding and helpful. Case notes are to be kept current at all times. Record your process notes at the end of each session or day of sessions. Process notes should include information about the client’s presentation in session, affect, and material discussed. Notes should document clinical observations, interventions, and progress toward clinical goals. (We will review assessment, treatment planning and, note taking). After the first three to four sessions an assessment and treatment plan should be completed. Your treatment plan should be updated every three months. At the end of treatment, a termination form with progress toward goals and further treatment recommendations should be completed. Keep copies of client intake forms, all releases, test results, letters, informed consent, and other client documents. All files must be kept at the counseling center at all times. Do not take files home. Files are the property of NVUSD and will remain with NVUSD at the end of your trainee/internship. If your client/patient goes with you to another location, the client may sign a release to have a copy of his/her file. The original will stay here. Maintaining confidentiality is both a legal and ethical responsibility for all clinicians. All information obtained in the therapeutic setting must be kept confidential except when a disclosure is mandated or permitted by law. The file cabinet where client/patient files are kept should always remain locked. You should not discuss cases outside of supervision, and no conversations for the purpose of consultation should take place with other interns or therapists in any public settings, i.e. office bathrooms or hallways. Answering machine messages should be played at a low volume. Message pads should be kept locked in the client file cabinet. A shredder is available next in my office for any documents that include confidential information, such as patient names and phone numbers. If anyone attempts to discover the identity of your clients or the clients of any other employees or counselors, explain that information about the identity of clients is confidential and that you cannot confirm or deny any information. Privilege is the legal right not to have confidential information revealed in a court proceeding. It is our duty to maintain the privilege of our clients unless the client waives it, or if a court orders us to produce information. 2425 Jefferson Street, Napa, CA 94558 | 707-253-6865 | www.nvusd.org If you receive a call from an attorney or if you receive a subpoena or other court document, take no action without first discussing the matter with me; and do not reveal the identity of any of your clients. If anyone attempts to discover the identity of your clients, explain that information about the identity of our clients is confidential and privileged and that you cannot confirm or deny any information. We are mandated reporters of child, dependent adult, and elder abuse. If you have knowledge of reasonable suspicion of abuse, it is your responsibility to notify the appropriate authority as soon as possible. If you are not sure whether to report, check with me as soon as possible. If you believe the information you have received is not reportable, check with me as soon as possible. Whenever you are faced with a situation in which it is necessary to break a client’s confidentiality, please check with me as soon as possible. Use the following number to contact me. 707-501-8737. We will train you on emergency protocol. We are also mandated to make Tarasoff warnings when necessary. If you believe your client presents an immediate danger to a specific person, then you must notify that person, and the police. Contact me as soon as possible, and before taking any action. If I cannot be reached, you will follow the directions regarding emergency protocol. Clients should be regularly monitored, and evaluated for danger to themselves or others. If a client presents a danger, attempt to intervene in a way that does not break confidentiality. If you need to break confidentiality, try to first check with me. Clients who are a possible danger to themselves or others should always be brought up in supervision so that we can adequately monitor those cases. In order for psychotherapy to be beneficial to our clients, it is important to maintain the integrity of the therapeutic relationship. Dual relationships, acting in two separate roles with a client, need to be avoided. For example, it is unethical to be your client’s therapist, and piano teacher. Bartering and/or paying a client for their professional services also compromise the therapeutic relationship and should be avoided. Do not utilize any client services, from accounting advice, dental work, stock tips, mechanical repair, Avon purchases, etc. All of these situations run the risk of obscuring professional judgment or exploiting the client, and must be avoided. It is the policy of this placement to decline gifts. Accepting gifts can create misunderstandings with some clients, so it is best to avoid accepting any gifts. If a client brings you a gift, explain the policy, politely thank them, and process their reason for bringing the gift. You can also review this policy in the first session along with your consent to treatment, and identification of your traineeship standing. Sexual contact or sexualized conduct with clients is illegal and unethical. It is never acceptable to engage in a sexual relationship with a client. Any sexual fantasies or sexual advances on the part of the client need to be reported to me right away, so that we can manage the situation appropriately. If you notice yourself becoming attracted to a client, or having fantasies about him/her, alert me so that we can work to manage the counter-transference in a legal, ethical, and professional manner. We require that you take out your own individual policy. If you do not already have such insurance, let me know and I will give you ideas about where you can call to obtain appropriate information. CAMFT is providing free Liability Insurance in their student membership. One of my goals as your supervisor is to be accessible and available and to help you to feel comfortable asking any questions you might have. I will make myself available to you any time, day or night, for emergencies and crisis situations. I ask that non-emergencies be reserved for supervision. Supervision is generally the best time to talk to me because you will have my undivided attention. I look forward to our work together and expect both of us to learn and benefit from our professional association. I have read, understand, and agree to the policies and procedures. This document is an example of the supervisory consent form to be signed by you, and your supervisor. Thank you for your interest in our program.
