W.I.T.T.S Training Academy LLC.
Whatever It Takes To Succeed, LLC (W.I.T.T.S). uses various resources to come up with skill sets every athlete
needs to work on. Identifying the skill set is just the beginning. Our current focus is to serve youth interested in
basketball and football. We have come up with ways to equip each athlete with tools to improve each skill
resulting in better performance and notable improvements at the end of camp. Our camp coaches have won state
championships, played college and professional basketball & football, currently coach, and train professional,
elite high school/college, and youth sport athletes. Participants will benefit from the years of dynamic
experience our camp coaches have with athletes of all ages and ranges of skills.

Medical Release& Liability Waiver
I do hereby grant permission to W.I.T.T.S. LLC and their respective agents and employees to secure such medical aid and hospital services as
they deem necessary for the child noted on this form in the event he/she should sustain injury or illness while attending a W.I.T.T.S. LLC
program. I agree to assume the cost for transport and medical information of which the camp should be aware in consideration of the child’s
physical and mental well - being. The participants attending W.I.T.T.S. LLC/MRH School District and uses said equipment at his/her risk. By
signing this release, the parent/legal guardian knowingly waives the right to litigation if injury occurs out of said participation including but not
limited to the camps own negligence. W.I.T.T.S. LLC and MRH School District shall not be liable for any damage arising from personal injuries
sustained on or off the premises of said MRH School District.
