m. What are the Organization’s programs and activities? Children's Home Society
of Florida – Mid-Florida Division offers:
a. Community School, is both a place and a set of partnerships between
the school and the community designed to support students and
neighborhoods. The Howard Bishop Middle School community school is
a hub, bringing together many partners, (Alachua County School Board,
Health Department of Health in Alachua County, University of Florida,
Santa Fe College and Children's Home Society of Florida) to support
families and youth. The Community School is working to unite the most
important influences in a child's life - school, families and communities to create a network that supports their development toward productive
adulthood. Its integrated focus on academics, health and social services,
youth and community development and community engagement has
been shown to improve student learning, strengthen families and
contribute to healthier communities. Community schools offer a
personalized curriculum that emphasizes real-world learning and
community problem-solving. Schools become centers of the community
and are open to everyone.
b. Family Connections, is an evidenced based prevention program rooted
in Safety Methodology that seeks to prevent child abuse and provide
tools for the parents. It is a voluntary program with a family centered
approach to empower families to address areas of risk for improvement
and change. This program serves families with children who, while
deemed safe at the conclusion of the Child Protection Investigator’s
investigation, remain at high or very high risk of future involvement with
the Child Welfare System.
c. Mid-Florida Community Counseling, is a program designed to bring
clinical interventions to clients in their homes and schools. Most clients
with Medicaid coverage are eligible for these services. Our clinical team
is versed in a variety of trainings and clinical interventions to meet the
needs of their clients.
d. Healthy Families, is a program which is part of Healthy Families
America, a prominent, national child abuse and neglect prevention
program that recognizes the impact a family’s dynamic has on a child’s
environment. The program’s philosophy shares Fredrick Douglas’s belief
that “It is easier to build strong children than to repair broken adults.”

Healthy Families melds social services, education, and health care “best
practices” into a model that empowers families to become nurturing and
self-sufficient.
e. Family Treasures Thrift Shop, a program that serves the dual purpose
of raising funds for Children's Home Society of Florida programs and
providing items for the children and families served.
f. Family Visitation Centers, these programs provide safe environments
where children and their families can spend time together during courtordered supervised visits. Here, caring caseworkers demonstrate positive
role modeling for parents while ensuring children are safe. Additionally,
these centers allow divorced parents to exchange their children in a
neutral setting.
n. Does the Organization control, or is it controlled by, any other organization? If
yes, please explain. No, Children's Home Society of Florida does not control nor
is it controlled by any other organizations.
o. Is the Organization financially accountable to any other organization? If yes,
please explain. No, Children's Home Society of Florida is not accountable to
any other organization.
p. What assets does the Organization have that are used in the performance of
its functions? The Organizations assets consist of computer equipment, a
van, and office furniture.
q. What is the Organization’s annual budget? 16/17 Fiscal Year is $7,099,901.

THE PROJECT
a. What is the Organization’s Project? The Family Visitation Center is housed in a
home like setting in Gainesville and provides family in Alachua County a safe
place for children and parents to visit with each other. The home is 56 years
old and in need of renovations, repairs and safety improvements. With funds
from the Rotary we will be able to make the needed improvements which will
impact the building structure, function, and aesthetics of the home to provide a
nurturing, welcoming and warm environment for the families. Please see the
attached list of items/repairs needed to make the Family Visitation Center home
safer and more welcoming.
b. If the Project is funded, how would Rotary be recognized? Published on CHSMF Website, Unveiling with Rotary Members of the remodeled building with
press release issued to invite media. Acknowledgment in the Children's Home
Society of Florida e-Newsletter.
c. What is the geographic area the Project will serve? (e.g., the City of Gainesville,
or Alachua County, or the City of High Springs, etc.) The Family Visitation

Center offers families in Alachua County a safe place for the children to visit with
their non-custodial parents, usually subject to court ordered or mediated visits.
d. How does the Project benefit, improve or serve the Alachua County community?
With funds we will be able to repair the Family Visitation Center. The flooring,
fixtures and equipment are badly outdated and in need of repair or replacement.
Updating this home will provide a safer environment for families to visit and
ensure the staff are able to monitor the families while ensuring the safety of
children. The repairs will benefit the Alachua County community because the
Center will be more welcoming to families in need of this service. The security
system will be expanded to include monitoring of the external building, thus
ensuring the safety of children and parent victims of domestic violence.
e. Does the completion of the Project depend on receipt of this Grant? Yes it does.
As we receive funding, we will continuing to seek the remaining balance needed
to complete the project for the Family Visitation Center.
f. How many people in each of the following categories will benefit from the
Project? We do not collect data on the first three categories. However, many
families are economically disadvantaged, socially disadvantaged and may
include disabled youth.

5. Senior Citizens – A few families served include grandparents.

USE OF GRANT FUNDS
a. How much Grant money is the Organization requesting for its Project from the
Foundation? $39,522
b. Is this Grant the major funding source of the Project? Yes, at this time this grant
is the major funding source of the Project. However, we continue to research
and apply for other funding sources such as community partners and corporate
donors.
c. How will the Grant funds be used? The Grant funds will be used to insulate the
home, remodel the inside of the Family Visitation Center, update the Security
System, and repair the damage to the outside of the building from years of
weather impact. The home is 55 years old.
d. What are the other funding sources of the Project, and what commitments have

you received? At this time, there are no other funding sources or commitments
for this Project. CHS has submitted applications to Lowes, Home Depot, and
the Mazda Foundation to help support this project.
e. What is the total budget or cost of the Project including all sources of funds?
$39,522
f. What is the timeframe for starting and completing the project? This project will
start once informed that we have been awarded the funds. We plan to complete
the renovations no later than one year from date of funding from this application
approval.
