August 29, 2016 MEMORANDUM TO: HOSA Chapter Members and Advisors FROM: Tate Ozbirn, HOSA State President and Dana Stringer, HOSA State Advisor RE: 2017 HOSA STATE QUALIFYING EVENTS (SQE) NEW THIS YEAR: On-line testing and digital submissions will be used for members to qualify for competition at Alabama HOSA’s State Leadership Conference (SLC). All competitions, with the exception of Recognition Events and Special Needs Events, MUST qualify through this process to advance to SLC. ACTIVITY SQE Registration opens DEADLINES AND FEES August 30 Proctor Agreement Forms October 15 AND Advisor Agreement Forms Due On-line Proctor Identification October 15 SQE Registration Deadline: November 1 by 11:00 pm SQE Registration fees $10.00 dollars per competitor SQE Registration payment PAYMENT MUST BE MADE OUT TO ALABAMA HOSA AND POSTMARKED BY NOVEMBER 1ST SQE Testing/Submission window SQE Results National HOSA Week, November 6-12, 2016 By November 22, 2016 SLC Registration Deadline December 13 NOTES www.hosa.org, login as an advisor, and register affiliated HOSA members through the conference system. Proctor Agreements must be digitally submitted. Anyone that might proctor your students needs to complete an Agreement. Proctors will receive an email near the testing window with detailed instructions. Advisor Agreements must be digitally submitted for EACH advisor associated with your school’s Health Science Program. Last day to identify proctors online. Everyone that MIGHT proctor your students needs to be submitted online at www.hosa.org and login. LAST day to register members for SQE events. Late registrations will not be accepted. Qualifying competitors will be posted on the www.AlabamaHOSA.org website. SQE PREPARATIONS CHECKLIST Affiliate members Set chapter deadlines for registration and payment (State deadline is November 1) Collect student payment before your registration deadline ($10.00/competitor) Secure a testing site that will accommodate the number of students you will be testing each day during the testing window Make sure your school computers can access the testing site: http://www.hosa.org/hosaconf (link will only be active during the testing window) Secure a proctor that is NOT associated with HOSA or Health Science (suggestions are librarians, counselors, IT staff, administration, etc.) Secure 1-2 back-up proctors in the event your primary proctor is unavoidably absent during your testing window. Have each identified proctor sign the Proctor Agreement form and every advisor sign the Advisor Agreement Form in this packet and submit to [email protected] by the deadline of October 15 Submit information to “Identify Proctors for Online Testing” by the deadline of October 15 (you will need their email addresses) o http://apps.hosa.org/hosaconf/login.jsp, login as local advisor, click “Identify Proctor for Online Testing” o Without this step, your competitors will NOT be allowed to test Double check your registration in the on-line system o Correct events and students o Teams are labeled correctly and are complete Each team event begins with Team 1 For example, if you have three teams in one event; you will have teams 1, 2, and 3 in that event If you have five teams, all in different events, each team will be labeled as Team 1 o Remove any students that drop out PRIOR to the SQE deadline o There will be no refunds for students that fail to test/compete Once you confirm your registration information, print your invoice, and mail payment to our NEW BUSINESS OFFICE ADDRESS below, postmarked on or before the deadline o Alabama HOSA Business Office PO Box 912 Jacksonville, OR 97530 Confirm your testing dates with your proctors and your testing sites (Suggestion: do not wait until the last day of the testing window in case there is a problem with computer access) Make sure students taking the same test are all testing at the same time o You may test over several days, but all students taking a particular test must test at the same time o For example: All medical terminology competitors must test on the same day, while medical spelling competitors can all be tested the next day Assure that all proctors have received email login instructions and a proctor script by November 6 o Without this documentation in hand, they will not be able to log your student into the competition Provide proctors with blank copy paper and BASIC calculators (not cell phones) for Medical Math competitors Assure that all students in competitive events requiring digital submissions have provided Alabama HOSA the required competitive sample by the deadline of November 12 to [email protected] o Digital submission requirements for non-testing events are listed later in this packet o A confirmation email will be sent after each submission received by Alabama HOSA Check www.alabamahosa.org for SQE results o See the possible qualifying numbers for each event on the following page Use the link on the www.alabamahosa.org website to print a certificate of participation for each competitor Use the link on the www.alabamahosa.org website to print certificates of excellence for each qualifier Digital submission requirements for the non-testing events: Submissions should be made to [email protected] All submissions should include the following in the subject line: Event Title / Student Name / School Name Public Health Extemporaneous Health Poster Health Career Photography Job Seeking Skills Prepared Speaking Health Career Display Health Education Public Service Announcement Medical Innovations Follow round one directions in guidelines Submit a YouTube link Complete on letter sized copy paper. Proctor will provide 60 minutes after providing the topic. Topic is provided to proctor prior to testing week. Submit pictures via a link or by email. Submit 3 pictures via a link or by email. Label each picture: student name, school, & pic # Submit resume’ via email. Label attachment by student & school name. Submit a YouTube link to a two minute video clip of the student’s speech. First 2 minutes ONLY. Submit pictures of display via link or by email. 6 pictures maximum. 3 of the pictures must show measurements of the display (height/width/depth). Submit a YouTube link of a video of the team describing bullet one (the goal of their instructional topic, instructional planning process, and why they did what they did). No more than TWO MINUTES Submit the PSA video only, via YouTube or Google link Maximum of three pictures of display and one written page covering the content of 2A and 3A on the guideline’s rubric. Healthcare Issues Exam will ONLY be offered during the State Qualifying Event. This test will NOT be offered again at State Leadership Conference (SLC). The top scorers will be posted on the www.alabamahosa.org website and will be recognized at SLC during the Recognition Session on Friday. 2. There will be NO SQE testing for special needs and recognition events – ALL entries attend state for the following events: HOSA Chapter Reflections Healthy Vitals HOSA Happenings Banner Parade Interviewing Skills Personal Care Barbara James Service Award with > 50 hours National Service Project 3. Testing times outside of the posted testing window will NOT be allowed. Please schedule testing early in the window to allow for unexpected challenges. 4. For team events, if a student is sick on the test date the entire team must agree to test the next day or the absent member must forfeit. Time will not be allowed outside of the testing window. 5. Test sessions will be timed in accordance with HOSA guidelines. Timing begins when the test items are presented. a. 1 hour for 50 question tests and 1.5 hours for 100 question tests 6. Please review the use of scroll bars with competitors PRIOR TO TESTING. Remind them not to close the testing window (do NOT click the “X”). Closing the testing window terminates the test WITHOUT scoring and DISQUALIFIES the competitor. 7. Once a student logs into a test, the test must be completed in that sitting. There will NOT be an opportunity to save data and complete the test later. 8. If the proctor has difficulty submitting a test, do NOT close the test. I hereby agree to maintain the confidentiality of all testing materials and understand that the security of testing materials is preserved by protecting all items from loss, unauthorized access, or reproduction. I agree to fulfill the following duties: Assure the identity of the student taking the test Assure that all students/teams participating in the same event, compete at the same time Assure that student/teams test ONE time for each event Assure that students do not have access to any study materials, notebooks, papers, or phones Maintaining the security of the proctor pass codes and not allowing code access to students or advisors Maintaining the security of the testing environment by assuring advisors or other Health Science related personnel are not present during testing Assure the students are not allowed to share information while testing Assure that MEDICAL MATH competitors have 2 blank sheets of copy paper and a BASIC calculator Collect and discard Medical Math scratch paper after testing Refrain from making or allowing unnecessary noises during testing. For example: gum chewing, tapping on tables, bell ringing. Assure I have access to a phone (placed on silent) in the testing room to allow contact with Alabama HOSA in the event of testing problems. I will not allow the following: Unauthorized printing or photocopying of any test items or materials Accessing or removing test materials from the secure locations Utilize test items in any form Student access to events at times other than their scheduled test times Advisors or others to view test content I understand that access to testing will be monitored by Alabama HOSA and inconsistencies will be investigated. I further understand that compromising test security in any way may result in the disqualification of all HOSA members testing at this school. I also understand that it is the responsibility of the testing institution to handle IT issues promptly prior to and during testing. I hereby agree to maintain the confidentiality of all testing materials and understand that the security of testing materials is preserved by protecting all items from loss, unauthorized access, or reproduction. In order to maintain the integrity of the testing and competitive process, I further understand that advisors are not allowed to access tests, enter the testing room at any time for any reason, or view tests at any time. Advisors are not allowed to question proctors about test content. Maintaining test item security prohibits any advisor or test proctor from the following: Unauthorized printing or photocopying of any test items or materials Accessing or removing test materials from the secure locations Utilizing test items in any form Student access to events at times other than their scheduled test times Viewing, or allowing others, to view test content I agree to fulfill the following duties: Securing a unbiased proctor that does not have any affiliation with the Health Science/PLTW Biomed class Securing substitute proctors to replace the primary proctor in the event of an emergency Documenting the proctors using the online system prior to the deadline Assuring that all students/teams participating in the same event, compete at the same time Collaborating with proctor and testing site to schedule all competitive events during the testing window Maintaining the security of the testing environment by prohibiting access to anyone other than competitors, proctors, and IT staff (if needed) Assuring that students understand the testing rules in relation to honest and fair competition Providing the proctor with 2 blank sheets of copy paper and a BASIC calculator for each Medical Math competitor I understand that access to testing will be monitored by Alabama HOSA and inconsistencies will be investigated. I further understand that compromising test security in any way may result in the disqualification of all HOSA members testing at this school. I also understand that it is the responsibility of the testing institution to handle IT issues promptly prior to and during testing.
