CASE WORKER JOB DESCRIPTION
POSITION: Case Worker
EMPLOYER: Southern Sydney Women’s Therapy Centre
RESPONSIBLE TO: The Manager/Assistant Manager
STATUS: 18 hours per week
SALARY: Level 4 Pay Point 3 SCHADS Award
CONDITIONS: SCHADS AWARD and SWCC above award provisions.
This position is funded through The Canterbury Bankstown
Multicultural Family Homelessness Support Service (Linking Hearts
Project) within which SWCC is a consortia partner.
Position Summary
The purpose of the case worker’s position is to assist vulnerable
families to avoid the risk of homelessness and assist people
experiencing homelessness to break the cycle of homelessness.
Priority client groups for case management are people from
culturally and linguistically diverse backgrounds including
 Women and Children escaping domestic/family violence
 Men and/or women with children
 Refugees and those from new or emerging communities with
children
The case worker is required to work with the partner agencies to provide
comprehensive service delivery ensuring




early intervention/prevention
rapid housing responses
crisis responses
intensive responses for clients with complex needs
Key Responsibilities include
 Working in a manner which complies with the set protocol and
guidelines of the project.
 An understanding of the GHSH reforms
 Ensuring a collaborative and respectful relationship with clients
 Working collaboratively with team leaders
 Delivering client assessment and/or collaborative assessment for
best service fit to meet the needs of clients

Responding to referrals and making referrals as required to
partner agencies and other services providers
 Maintaining a collaborative co case management partnership
with partner agencies and other services providers to achieve
optimal outcomes for clients
 Maintaining regular contact with allocated clients to
o Assess, plan and review client goals and strategies
o Assist clients to access income support, legal aid/ court
support
o Assist clients with safety planning/AVO’s if required
o Assist clients to access counselling/mental health services/
FACS etc if required
o Assist with housing transition planning
o Advocate on behalf of clients as required
o Attend case conferences
o Provide outreach services including accompanying clients
to meetings as required
o Provide aftercare support to transitioned clients
o Facilitate the use of brokerage funds as required
 Engage in early intervention/prevention awareness raising
activities regarding homelessness and associated complex issues
 To maintain a positive working relationship with the Canterbury
Domestic Violence Committee and participation in projects
developed through the Committee.
 Ensure the accurate recording of all statutory and client records
in keeping with policy and procedures relating to service delivery
and evaluation of outcomes
General
 To attend and actively participate in staff and planning meetings.
 To participate in the quality improvement process of the Centre and
service provision.
 To keep and provide statistics and other data as required to enable
the ongoing evaluation of the service.
 To contribute to the Annual Report.
 To participate in quality improvement processes and accreditation
reviews.
 To participate in professional supervision and the professional
development program provided by the Centre.
 Attend Line Management
 Complete an annual performance appraisal
 Adhere to the Centre’s Work Health & Safety Policy to ensure the
health and safety of all in the organization, including other staff,
clients, volunteers, students and visitors to the Centre.
 To fulfill administrative requirements of the position such as
completion of time sheets, travel claims etc. in accordance with the
Centre’s policy and procedures.
 To perform other such tasks as may be determined by the Manager.
Person in this position is required to carry out duties in the spirit of
the philosophy of the Centre as expressed in the vision and policy
statements, and as a member of the Centre’s team.
