How to create your own language version of the Debtless Attitude Survey in
Google Docs Forms
1) Go to: https://groups.google.com and sign in.
2) Select ‘Documents’. You may find it has another name in your country’s version.
3) Click on ‘CREATE’:
4) Select ‘Form’ and you are ready to get started!

6) Click on ‘Theme’: change it from ‘Plain’ to ‘Budget’ and click ‘Use’ top left corner.
7) Now add your translation of the Title and Introductory information below in the top two
boxes – ‘Untitled form’ & the box underneath: ‘You can include any text or info ...’
8) Now for the questions. Click on ‘Add item’ in the top left corner.
All questions use Question Type: ‘Checkboxes’.
Just add your translated text into the boxes.
9) Here is a typical ‘yes / no’ question.
NOTE: I only clicked: ‘Make this a required question’ for the ‘About you’ questions.
These are the only mandatory questions.
10) When you have typed or pasted in your text for a question, click ‘done’.
11) If you need more checkboxes – for example in Question 7, just click on the bottom line:
‘Click to add option’.
12) After Question 20 a different question type was used for the concluding section – ‘Scale’:
13) Just check that the word ‘Saved’ is showing top right when you are finished.
If it still says ‘Save’ click it. Most of the time it updates automatically but just to be sure ..!
14) You can now see what your survey looks like by clicking on the link at the bottom of the
screen:
