2016-17 NCYSA Guest Roster for Out of Association Player
This guest player form is for NCYSA registered players guest playing with a NCYSA team in a sanctioned tournament, practices, scrimmages or friendly games. The coach of the guest team is
responsible for the proper completion of this form, or risk being found guilty of recruiting. Passed by BOD 6-6-99. Only non-Association guest players are required to complete a NC Guest Roster.

Note: This form is only for NC players playing with NC teams. NC players playing out of state, or out of state players playing in NC, must complete the Interstate Permission Form.
INSTRUCTIONS:
1. Complete all required information. If guest players come from multiple teams, a form must be completed for each team. Both the seasonal (home coach) and the guest coach must sign form
2. The registrar for the seasonal (home) association must red stamp the form directly below the last player listed. Form is not valid without home registrar’s stamp.
3. Parent of guest player must complete a new NCYSA Medical Waiver for the guest team.
4. Player will use their seasonal pass and it is the PARENT”S responsibility to return the pass to the home team’s manager within 5 days following end of event.
