What is District Blitz?
The best weekend of the year! District Blitz in a state-wide EFCA youth conference held once a year.
It’s a weekend to hang out with friends and learn more about God. We play games, swim, eat, hang
out, explore Duluth, and attend four awesome rallies! Each rally includes worship, prayer, and
messages to challenge students on what a real relationship with Jesus actually looks like in their
personal, everyday lives. There are also breakout sessions on important topics, and the students pick
which to attend based on what sounds interesting to them. It’s a great opportunity to connect with
friends, leaders, and The Lord. We want you to come with us!
Who can attend?
Students 5th grade through 8th grade are invited to attend with us, even if they’re not a part of our
youth group. Please note that registration is broken into blocks, so students will register at different
times.
When is it?
April 21st-23rd
Students will gather at 2:30pm on Friday, April 11st to check in and get everything ready to head out.
Parents are invited to drop luggage off at the church early that day if they wish and have students walk
to PVC after school.
We will return at approximately 5pm on Sunday, April 23rd. Please plan on meeting your student at
church at that time.

How much?
$150 – this amount includes food for the weekend, lodging at a great hotel (with a WATER PARK!),
travel, and conference fees. If this is more than your family can afford at this stage, please contact
James ([email protected]) to discuss ways to raise support or receive a scholarship – we don’t
want anyone to miss out because of cost!
When do I register?
Because we have limited seats available, registration will occur in three stages. Once we are full,
students will be added to a waiting list.
Sunday, March 5th – Saturday, March 18th: 7th and 8th grader pre-registration.
Sunday, March 19thst – Saturday, April 1st: All youth group student registration. We want to
make sure our students get a chance to go before friends fill up the registration. Any student who has
attended Sunday School or Wednesday Night Youth Group once in the last three months may register
during this time.
Sunday, April 2nd – Saturday, April 15th: Open Registration. Any students, 5th-8th grade, may
register during this time.
Sunday, April 16nd – Thursday, April 20th: Late Registration. Registration fee becomes $180.
How do I register?

registration information.
2. Submit “Medical Release and Permission Form” (Pleasant Valley’s release form)
3. Submit “District Blitz Medical Release.”
4. Submit “District Blitz Code of Conduct.”

Medical Release Form (student)
District Blitz Conference, April 21-23, 2017
Because of the increasing sophistication of our hospital systems, we have found it necessary to have signed parental
release forms in the unlikely event of some serious injury requiring hospital treatment. Since many hospitals will not
administer any medical treatment to a minor without some parental consent this release forms gives us permission to take
your child to the nearest available medical facility and have the necessary medical treatment administered.
Please read and sign the statement below. This provides permission to seek whatever medical attention may be
necessary. It also releases North Central District of the Evangelical Free Church of America, CNC (Converge North
Central), Trout Lake Camps and / or the church's personnel from any liability against personal injury or loss.
We understand the arrangements and believe that necessary precautions and plans for the care and supervision of the
child will be taken during this conference. Beyond this, We will not hold responsible the North Central District of the
Evangelical Free Church, CNC (Converge North Central), Trout Lake Camps or any of the conference staff. In case of
emergency I understand that every effort will be made to contact me. If I cannot be reached, I hereby give the Student
Ministries Conference leadership, staff or other emergency medical personnel the permission to act on my behalf in
seeking emergency medical treatment for this child in the event that such treatment is deemed necessary by the
conference or church staff. I give permission to those administering emergency medical treatment to do so using those
measures deemed necessary. I absolve the North Central District of the Evangelical Free Church, CNC (Converge North
Central) and Trout Lake Camps and/or church personnel from Liability in acting on my behalf in this regard so long as
they are not grossly negligent.

Conduct Pledge
We want to remind each youth leader and student that we are representing Christ in Duluth. Therefore, our speech and
conduct should be glorifying to God (I Tim. 4:12; Col. 3:17). Please discuss this page with your group before the
conference. We are looking forward to the best District Blitz conference ever, and hope that you will be a part of making it
happen! Your cooperation is vital and greatly appreciated.
Conduct Code
 My model for conduct and attitudes is the Lord Jesus Christ.
 I will observe all rules established by the Conference directors and will follow the code given by my
church.

For my own safety sake, I will not leave the rally area unless given special permission by my
Youth Pastor/Leader.

I will attend all scheduled rallies and sit with my group.

There will be no hand holding or arm around the opposite sex.

Any free time given me will be used wisely, and I will, at all times, respect the property and
safety of others.

No members of the opposite sex are allowed in hotel rooms.
 I will (in travel to and from the rally, and during the conference) avoid all questionable activity and things
which would be offensive to Christ, my parents, my church, the rally and others.
Responsibility Pledge
 My goal in attending is for spiritual growth and to enjoy and develop friendships. I accept the
responsibility for my conduct at the District Blitz Conference.
 I realize that my participation in questionable behavior will end my stay at the Conference.
 I understand that my parents/guardians will assume financial responsibility due to property damage/loss
caused by me and travel cost if I am asked to leave early (only after parents have been called).
 I will not bring a radio or CD/Mp3 player or other digital audio and video devices to the Conference.
 My family and I will be praying that this conference will be an enriching experience for our entire youth
group.
