Acknowledgment of Participation
Parents / guardians and players should be aware of the possible risks that are inherent in the nature of some of the
activities. These risks include, but are not limited to, the potential for accidents or illness while participating. Every attempt
is made to minimize the existing risks through proper sports equipment, safe facilities and sound safety practices.
However, parents / guardians and players should realize these risks cannot be eliminated.
By affixing including the player’s name to the team roster, the player and the parents / guardians understand that
the player is participating in the 2017 YFL Football Season at the player’s own risk and hereby agree, indemnify and hold
harmless the Youth Football League Associations and the coordinators of the tournament from any liability in connection
with the activity.
In addition, by affixing the player’s name to the team roster, the player and the parents / guardians understand that
the Youth Football League Associations carry no medical or accident insurance that will benefit the player, should the player
be injured during the course of the competition.
Quit Claim and Waiver
I, the parent / guardian of the registering player, affirm that I am aware of my son’s / daughters physical condition
and I he / she is voluntarily participating in the 2017 YFL Football Season. I am aware that such participation may result in
possible injury as a result of his / her involvement in the sport. In addition, I hereby release all the playing venue
managements of any and all responsibility in case of any personal injury sustained by my son / daughter or damage to
property of others caused by my son / daughter while participating in the football activities. I have read and understand the
above statements and will abide by them.
Conformity to the Rules and Regulations
I, the parent / guardian of the registering player, hereby state:
1.
That my son / daughter is a player of the abovementioned registering club participating in the 2017 YFL Football
Season as indicated in this duly accomplished and submitted Player Registration Form;
2.
That I have read the official rules and regulations governing the conduct of the 2017 YFL Football Season as well as its
annexes or that the same has been read to us in a language that we understand;
3.
That I fully understand the obligations and duties that are placed upon our son’s / daughter’s club and upon each of us
as parents / guardians in the 2017 YFL Football Season;
4.
That I, as shown by my signature hereunder, covenant to allow my son / daughter to attend and participate fully in all
official functions and activities, official matches and media engagements connected with the Season and shall arrive
in a timely manner and remain until all official activities are completed;
5.
That I, as shown by my signature hereunder, allow my son / daughter to be bound by the official rules and regulations
of the 2017 YFL Football Season as has been and may be promulgated by the organizers;
6.
That I impose these obligations upon my son / daughter willingly and voluntarily and without mental reservation or
purpose of evasion.
