Nomination Award Criteria & Submission Form On May 18, 2017, 211 Helpline is excited to present its 3rd Non-Profit Awards, Jewels of the Treasure Coast Luncheon! We will once again have the opportunity to not only recognize the outstanding work being done by nonprofit agencies in Indian River, Martin, Okeechobee & St. Lucie Counties, but also to celebrate and reward their commitment to serving individuals in need. The luncheon will take place at the Santa Lucia River Club at Ballantrae in Port St. Lucie—a centralized location for all four counties! We are expecting over 200 non-profit staff, board members, community volunteers, individual donors, government officials, foundations and corporate funders to come together on May 1st to celebrate the true “Jewels” of our community. 211 HelpLine is now accepting nominations for Non-Profit Organizations that demonstrate exceptional efforts within the communities they serve in ten categories: Non-Profit Organization Leader of the Year – Treasure Coast Non-Profit Organization Board Leader of the Year – Treasure Coast Non-Profit Organization Volunteer of the Year – Treasure Coast Non-Profit Organization of the Year, Animal Welfare – Treasure Coast Non-Profit Organization of the Year, Rising Star – Treasure Coast Non-Profit Organization of the Year – Indian River County Non-Profit Organization of the Year – St. Lucie County Non-Profit Organization of the Year – Martin County Non-Profit Organization of the Year – Okeechobee County Community Choice Award – Treasure Coast Up to three finalists will be selected in each category by a panel of impartial judges. Of the finalists, one winner in each category will be announced at the event on May 18, 2017. Winning organizations will receive a personalized award at the ceremony. In addition, winning organizations and finalists will receive a numerous amount of publicity and awareness of their work through event press, media materials and online & social media coverage. Award Criteria Nominations open January 16, 2017 and are due by 5pm on February 17, 2017. Finalists will be announced March 17, 2017. Winners will be announced (at the event) on May 18, 2014. All nominees must meet the following criteria: Organizations must be 501(c)(3) organization. Organizations must be physically located in the Treasure Coast with the majority of activities serving Treasure Coast residents. Organizations must be registered, listed and have updated information in 211’s Community Resource Database. Visit www.211treasurecoast.org/add-or-update-information for more information on adding or updating your organization. Organizations are encouraged to self-nominate or may be nominated by others. Organizations may be nominated in more than one award category but must submit a separate nomination form for each category. Every nomination must include the completed Award Submission Form Applications can be no longer than 3 pages. A copy of the organizations 501(c)(3) letter must be included with the application. Finalists may be required to provide additional information, including copies of the latest audited financials or 990 form. All finalists will receive notification via email. At least one representative must be available to attend the luncheon on May 18, 2017 at the PGA Country Club (only $35 per ticket for non-profit staff). Finalists in each category will be required to submit additional pictures of their organization in action. Further instructions will be given to each finalist. Award Categories Below are the ten award categories and the specific criteria for each. Nominations should follow the criteria for the category in which the individual/organization is being nominated. Non-Profit Organization of the Year, Rising Star – Treasure Coast This award will be given to a non-profit organization that is young and small but has the potential to grow and make a significant impact on the community it serves. Has been in existence for five years or less Has a clearly defined mission and vision for its future Demonstrates a cohesive board/staff partnership and strong leadership Provides a vital service to our community Non-Profit Organization of the Year, Animal Welfare – Treasure Coast This award will be given to an organization that demonstrates a commitment to improving and protecting animals’ lives in a humane manner. Displays a strong track record of savings animals’ lives in the community Has a clearly defined organizational mission and vision Implements humane policies and procedures Demonstrates the ability to secure community and funding support Non-Profit Organization Leader of the Year – Treasure Coast This award will be given to a non-profit leader (Executive Director/Board President/CEO) from an organization that has a proven track record of exceptional leadership and accomplishment within their organization. Demonstrates visionary leadership Transforms challenges into opportunities and success Exhibits leadership and facilitates collaboration in the broader community Demonstrates creativity, innovation, and/or entrepreneurship to successfully address core organizational or community issues. Non-Profit Organization Board Member of the Year – Treasure Coast This award will be given to a non-profit Board Member from an organization that has a proven track record of enhancing the mission and visibility of the agency. Demonstrates a strong commitment to the mission and vision of the organization Models effective Board and community leadership Engages self and other in obtaining funds/resources for the organization Receives no compensation from the organization Non-Profit Organization Volunteer of the Year – Treasure Coast This award will be given to an outstanding volunteer whose unselfish and dedicated service to the agency they serve has made a significant difference in the organization. Acts as an ambassador for the organization Has a track record of furthering the agency’s mission Receives no compensation for the organization Organization of the Year – Indian River, Martin, Okeechobee & St. Lucie (4 different awards) These awards will be given to non-profit organizations that have a proven track record of positive impact in the community, outstanding performance and financial sustainability.
