Letter from CSO manager to carer/s regarding outcome of investigation into possible abuse
or neglect

As you are aware, <CSO name>, in collaboration with the Department of Health and Human Services,
recently conducted an investigation into quality of care concerns related to possible abuse of a
child/children in your care. The reported concerns related to <brief description of reported
concern>. The investigation included an interview with you on <interview date>.
As part of determining whether any action was required, a panel was established to initially screen
the concerns. This panel consisted of <insert team manager name><insert role>, <insert CSO

Option 1
The allegation of abuse has been substantiated. This means Child Protection assessed, on the balance
of probabilities, it is more likely than not <physical abuse/sexual abuse/serious neglect/other> has
occurred.
The implication of this outcome is that <an action plan will need to be developed to ensure all
concerns are addressed and there will be no future risk to children in your care or it will no longer
be appropriate for clients of child protection to be placed in your care>.
The action plan will need to address <examples: behaviour management strategies; understanding
the needs and behaviour of vulnerable children; effective supervision; and managing personal
stress>. <CSO key contact name and position> will discuss the development of the action plan with
you.
Option 2
The allegation of abuse has not been substantiated. However, the investigation did identify quality of
care issues that we will need to discuss and work through to ensure they are addressed and there will
be no risk to children in your care. This process will need to address <examples: behaviour
management strategies; understanding the needs and behaviour of vulnerable children; effective
supervision; and managing personal stress>. <CSO key contact name and position> will discuss this
with you further.

Option 3
The allegation of abuse has not been substantiated. However, the investigation did identify quality of
care issues that will need to be addressed through a formal care review process. <CSO name> will
work together with you to develop an action plan to ensure all concerns are addressed and there will
be no future risk to children in your care.
The formal care review will need to address <examples: behaviour management strategies;
understanding the needs and behaviour of vulnerable children; effective supervision; and managing
personal stress>. <CSO key contact name and position> will discuss the process and action plan with
you.
Option 4
The allegation of abuse has not been substantiated and the investigation did not identify quality of
care concerns. No further action will be undertaken in relation to this matter.
