Official Rules
NO PURCHASE IS NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT
INCREASE YOUR CHANCES OF WINNING. ALL FEDERAL, STATE, LOCAL, AND
MUNICIPAL LAWS AND REGULATIONS APPLY. VOID WHERE PROHIBITED.
By participating in the Promotion (defined below), Entrant (defined below) confirms that
he/she has read and agrees to these Official Rules. Participation constitutes Entrant's
full and unconditional agreement to be bound by these Official Rules. Decisions of
Sponsor (defined below) are final and binding in all matters related to the Promotion. By
participating in this Promotion, all Entrants acknowledge that all information submitted by
an Entrant may be shared with Sponsor’s third party agents or vendors for purposes of
administering this Promotion and the prize.
Sponsor: The Promotion is sponsored by Mylan Specialty L.P., 1000 Mylan Blvd.,
Canonsburg, PA 15317 (the "Sponsor").
Promotion Period: The “EpiPen® On Location™” Promotion (the “Promotion”) begins at
12:00 a.m., Eastern Time (“ET”) on May 7, 2015 and ends at 11:59 p.m. ET on July 9,
2015 (the “Promotion Period"). Sponsor’s computer is the official time-keeping device for
the Promotion. All entries must be submitted and received during the Promotion Period.
Odds of winning depend on the number of entries received during the Promotion Period.
Eligibility: Promotion is open to all legal residents of the United States and District of
Columbia who are thirteen years or older as of May 7, 2015 (each an “Entrant” and
together “Entrants”). Entrants who are between the ages of 13-17 represent that they
have received permission to enter the Promotion from a parent or legal guardian. This
permission will be secured from the parent or legal guardian via the parental consent
form that is part of the entry process for the Promotion. The Promotion calls for each
Winner (defined below) to provide Sponsor with a 250-500-word testimonial about each
Winner’s experience during the prize trip (the “Winner Testimonial”) and Sponsor may
use photos or videos of the prize trip that are either taken by Sponsor or submitted by
Winner.
Employees, officers, and directors (including immediate family members and members
of the same household) of Sponsor, Disney Worldwide Services, Inc. (“Disney”), and
their respective parents, affiliated companies, subsidiaries, and advertising and
promotion agencies, suppliers, and vendors, and any other entities associated with this
Promotion (collectively, the “Released Parties”), are not eligible to participate.
Healthcare professionals (“HCPs”) as well as their immediate family members and
members of the same household are also not eligible to participate. HCPs includes any
person in a position to generate federal healthcare business including, without limitation,
Professional Healthcare Providers (any individual licensed by a state regulatory body to
provide patient care) and Non-Professional Healthcare Providers (any individual not
engaged in direct patient care) who are in a position to directly purchase, recommend,
use, prescribe, dispense, or to arrange for the purchase, recommendation or prescription
of one of Sponsor’s or its affiliate’s products used in the United States.

How to Enter:
During the Promotion Period, eligible participants will be invited to enter the Promotion
by visiting the Sponsor’s promotional website located at www.EpiPenOnLocation.com
(the “Website”). To enter the Promotion, Entrants must fully complete the “online
registration form” (the “Registration Form”) and submit it on the Website. Required fields
include first name, last name, email address, and date of birth. Promotion Winners will
be required to provide additional details, including Social Security number, for tax
purposes only. Incomplete Registration Forms will not be entered in the Promotion. If
Entrant is age 13-17, the page will redirect to a parental consent form that will need to be
completed by parent or legal guardian. Once an Entrant has fully completed and
submitted the Registration Form on the Website, the Entrant will be asked to share the
video on Facebook, Twitter, and/or in email using #Promo and #OnLocationTips. Each
click of a Facebook, Twitter and/or email share button will gain an Entrant one entry into
the Promotion. The Entrant can return to the Website each week throughout the course
of the Promotion to complete the Registration Form and share the video on Facebook,
Twitter, and/or in email using #Promo and #OnLocationTips. A maximum of three (3)
entries per week will be counted towards the Promotion.
Entrants may also enter via mail by printing on a 3”x 5” card, the Entrant’s name, date of
birth, address (including zip code) and e-mail address and mailing the card in a business
size envelope to: The “EpiPen On Location” Promotion, 90 Rockwood Place, Rochester,
NY 14610. All mailed entries must be postmarked by July 9, 2015 and received by July
15, 2015. Limit one (1) entry per envelope mailed.
Entries in excess of three (3) per person, per week will not be counted. An
individual may only use one personal or business email address to enter the Promotion.
An individual may not enter more than once by using multiple personal or business email
addresses. Multiple Entrants are not permitted to share the same email or other account
needed for participation. Any attempt by any Entrant to obtain more than the stated
number of entries by using multiple/different email or addresses, accounts, identities,
registrations and logins, or any other method will void the Entrant’s entries and that
Entrant will be disqualified. Entrants should enter using only an e-mail address
subscribed and registered in their name. Once submitted, an entry cannot be deleted,
canceled or modified. In the event of a dispute as to entries submitted by multiple
individuals using the same email account, the subscriber is deemed to be the natural
person who is assigned an email address by an Internet access provider, online service
provider or other organization which is responsible for assigning email addresses or the
domain associated with the submitted email address.

Drawing and Notification: Three (3) potential Grand Prize winners will be selected in a
random drawing that will take place on or about July 17, 2015 from all eligible entries
received. Odds of winning will depend on the total number of eligible entries received.
Potential winners will be notified by the email address that they included on their
registration form.
Winner Eligibility: Potential Promotion winners will be sent notification via e-mail from an
e-mail address formatted as [email protected], within three (3) days of the winner
selection or as soon thereafter as reasonably practicable. Potential Promotion winners
must respond to notification within ten (10) days of notification being sent. Entrant must
be able to travel between September 25, 2015 and November 16, 2015 and agree to
participate in the Winner Testimonial in order to be named a winner. In addition, potential

Promotion winners must sign an agreement (the “Agreement”) with Sponsor agreeing to
participate in the Winner Testimonial as described in these Official Rules, granting
Sponsor all necessary rights to publicize and use the Winner Testimonial and related
photos and videos taken by Sponsor or submitted by Winner, and releasing Sponsor from
liability related thereto, in addition to also signing an Affidavit of Eligibility. The potential
winners then and only then become the “winners.” If the potential winner cannot be
contacted within a reasonable time period, if the potential winner is ineligible, if any
notification is returned undeliverable, or if the potential winner otherwise fails to fully
comply with these Official Rules, then he/she will forfeit the Promotion prize and an
alternate winner will be selected from the remaining entries. Sponsor shall not be required
to attempt to select more than one alternate winner. Potential winner’s ability to travel as
part of the prize is a prerequisite to being named a winner.
Any potential Promotion winner selected must comply with these Official Rules, including
all criteria necessary for eligibility, to be a Promotion winner. Winning is contingent upon
fulfilling all requirements.
Prizes: Upon confirmation of potential Promotion winners eligibility, three (3) Grand
Prize Winners (“Winners”) will each receive a vacation package to Walt Disney World®
Resort near Orlando, FL, to visit the Epcot® International Food & Wine Festival (the
“Festival”) for Winner and up to three (3) guests (“Guests”) between September 25, 2015
and November 16, 2015. Prize package will include round trip coach class air
transportation for Winner and Guests between a major commercial airport near Winner’s
home (as determined by Sponsor in its sole discretion) and the Orlando International
Airport in Orlando, FL*; three (3) nights/four (4) days standard hotel accommodation
(one standard room, up to quadruple occupancy) at a Walt Disney World® Resort
Deluxe hotel; ground transportation between the Orlando International Airport and Walt
Disney World® Resort hotel via Disney’s Magical Express Service*; up to four (4) fourday Magic Your Way® Tickets with Park Hopper® Option; one (1) Disney Gift Card with a
value of $500; and an exclusive travel kit prepared by Nina Garcia.
* For a winner who is a Florida resident, air transportation may be substituted with
another method of transportation by Sponsor in its sole discretion and ground
transportation between the Orlando International Airport and Walt Disney World®
Resort hotel will not be provided.
Only Winners and Guests who elect to travel between October 2 and October 4, 2015
will also receive the following: an opportunity to attend a meet and greet at the Festival
with EpiPen® On Location Ambassadors Amanda Freitag and Auggie Maturo with his
parents; up to four (4) tickets to Sponsor sponsored culinary demonstration at the
Festival led by Amanda Freitag; and up to four (4) tickets to the Sponsor sponsored
Back-to-Basics food allergy awareness and dining preparedness session at the Festival
led by Auggie Maturo and his parents (exact time and date of all meet and greet
opportunities to be determined by Sponsor in its sole discretion.)
Approximate Retail Value (“ARV”) of each Prize is $6,155. Total Prize Pool ARV is
$18,465.
Actual prize value depends on the location of the Winner’s residence and dates of travel.
Travel must be taken between September 25, 2015, and November 16, 2015, and
must be reserved at least thirty (30) days in advance or prize will be forfeited and
Sponsor will have no further obligation to such Winner. If Winner elects to partake
in any or all portions of his/her prize with fewer than three (3) Guests or no Guests, the
prize will be awarded to Winner and each participating Guest and any remainder of the

prize will be forfeited and shall not be subject to further or alternative compensation. All
elements of the prize must be redeemed at the same time, and no changes will be
permitted after confirmation of any redemption. If Winner is between the ages of thirteen
(13) and twenty-one (21), he/she must be accompanied by an adult of at least twentyone (21) years of age. Each Guest must be of legal age of majority in his/her state of
residence, unless accompanied by a parent or legal guardian. Winner and Guests must
travel at the same time and must depart from the same city. ARV of prize may vary
depending upon the dates of departure, the points of departure, ground transportation,
and/or airline fare fluctuations; any difference between stated ARV and final ARV of
prize will not be awarded. Prize consists only of the elements expressly set forth above;
no other elements or expenses (including, without limitation, insurance, meals,
unspecified ground transportation, phone calls, gratuities, incidentals, souvenirs,
gasoline, etc.) are included in the prize and all such expenses are the sole responsibility
of Winner. Winner is responsible for any air travel taxes and/or expenses, including
applicable departure taxes or fees, inspection charges, and security charges. Once
issued, tickets are nontransferable and may not be reissued once travel has
commenced. Open tickets may not be issued and stopovers are not permitted. Each
Winner and his or her Guests are responsible for obtaining all travel related documents
prior to travel. Travel arrangements must be made through Sponsor’s agent. All prize
elements must be redeemed at the same time.
Prize Conditions: By accepting any prize, Winners agree to release the Sponsor and
the other Released Parties from any and all liability whatsoever for any injuries,
illnesses, death, losses, or damages of any kind caused by entering the Promotion or for
damages of any kind caused by any prize or resulting from acceptance, possession, or
use/misuse of prize awarded. No substitution or transfer of prize or election of cash in
lieu of prize will be permitted. Sponsor reserves the right at its sole discretion to
substitute any prize package with another prize of equal or greater value in the event
that the prize (or any component thereof) is not available. Each Winner is solely
responsible for all applicable federal, state and local taxes and/or any other fees/costs
associated with the prize or receipt of the prize. Accompanying parent or guardian must
be of legal age of majority in his/her jurisdiction of residence (and at least 18). The
Winners will be required to sign the Agreement and an Affidavit of Eligibility both of
which to be returned within 7 days of written notification, or prize may be forfeited and an
alternate Winner selected. If a Winner is a minor, then his/her parent or legal guardian
must be of legal age of majority in his/her jurisdiction of residence (and at least 18), must
also sign the Agreement and Affidavit of Eligibility to be returned within 7 days of written
notification, or prize may be forfeited and an alternate Winner selected. Winner’s Guests
may also be required to execute and return a companion liability release form. The
Winners will also be required to disclose his/her social security number for the purpose
of issuance of a 1099 Form for tax purposes. If a Winner is a minor, then his/her parent
or legal guardian must provide their social security number. Each Winner’s entry and
acceptance of the prize offered constitutes permission (except where prohibited by law)
for the Sponsor to use said Winner’s name, photograph, likeness, statements,
biographical information, voice and address (city and state), in all forms of media, in
perpetuity, without notice or further compensation. If a prize or prize notification is
returned as unclaimed or undeliverable to a potential Winner, or if a potential Winner is
not in compliance with these Official Rules, then such person shall be disqualified and
an alternate Winner will be selected for the prize at issue. If a Winner elects to travel
without Guests, no additional compensation will be awarded to the Winner and any
portion of the prize allocated to the Guests that is not used will be forfeited. Trip
restrictions/conditions stated herein are not all-inclusive and the trip may be subject to
additional restrictions/conditions, which may be stated in Winner/Guest travel documents

and the Agreement, affidavits and/or releases they are required to sign under these
Official Rules.
Use of Data: Sponsor will collect personal data about Entrants including phone numbers
and e-mail addresses in accordance with its Privacy Policy. Entrants may review
Sponsor's Privacy Policy at the following link: http://www.mylan.com/privacy-policy.
By participating in the Promotion and providing this information, Entrants hereby agree
to Sponsor’s collection and usage of their personal information and acknowledge that
they have read and accepted Sponsor’s Privacy Policy and, for purposes of
administering this Promotion and the prize, all Entrants acknowledge that all information
submitted by an Entrant may be shared with Sponsor’s third party agents or vendors.
Terms & Conditions: This Promotion is subject to all applicable federal, state, and local
laws. By entering the Promotion, Entrants agree to release, discharge, indemnify and
hold harmless the Released Parties, Facebook, and Twitter and each of their respective
directors, officers, employees, shareholders and agents from any and all liability, loss,
damage or injuries to any person (including, without limitation, death) or property of any
kind sustained in connection with their participation in the Promotion, resulting from
travel, from their acceptance, receipt, possession and/or use or misuse of the prize, or
for any typographical or other error in the printing, offering or announcement of the prize.
Entrant acknowledges that the Promotion is in no way sponsored, endorsed,
administered by or associated with Facebook or Twitter. WINNER MUST BE AWARE
THAT USE OF THE TRAVEL PRIZE CAN BE HAZARDOUS AND CAN PRESENT
UNUSUAL RISKS OF DEATH, SERIOUS ILLNESS AND INJURY, AND PROPERTY
DAMAGE. WINNER PARTICIPATES IN USE OF THE PRIZE WITH KNOWLEDGE OF
THE DANGER INVOLVED AND AGREES TO ACCEPT ANY AND ALL RISKS OF
DEATH, SERIOUS ILLNESS AND INJURY, AND PROPERTY DAMAGE.
Entrants further acknowledge that said parties have neither made nor are in any manner
responsible or liable for any warranty, representation or guarantee express or implied, in
fact or in law, relative to the prize, including, but not limited to, its quality or fitness for a
particular purpose. Any person attempting to defraud or in any way tamper with the
Promotion mechanics and/or implementation will be prosecuted to the fullest extent of
the law.
By participating, Entrants agree to be bound by these Official Rules and the decisions of
Sponsor, and waive any right to claim ambiguity in the Promotion or these Official Rules.
Sponsor reserves the right to disqualify any Entrant that Sponsor determines, in its sole
discretion, violates these Official Rules. Sponsor’s failure to enforce any of these Official
Rules shall not constitute a waiver of that provision.
Non-disparagement: Entrant agrees that she/he will not disparage and/or otherwise
negatively comment on the Released Parties’ reputation, business operations, products,
services, the Promotion and/or Entrant’s interaction with the Released Parties under these
Official Rules. This provision will not preclude Entrant from making truthful statements.
Internet Limitations of Liability: Sponsor reserves the right at its sole discretion, to
disqualify any individual who tampers with the entry process, and to cancel, terminate,
modify or suspend the Promotion in whole or in part, at any time, without notice.
Sponsor reserves the right to award prizes using all non-suspect eligible entries received
as of, or after (if applicable) this cancellation, termination, modification or suspension
date, or in any manner that is fair and equitable. Sponsor assumes no responsibility for
any error, omission, interruption, deletion, defect, delay in operation or transmission,

communications line failure, theft or destruction or unauthorized access to, or alteration
of, entries. Sponsor is not responsible for any problems or technical malfunction of any
telephone network or telephone lines, computer on-line systems, servers, or providers,
computer equipment, software, failure of any e-mail or entry to be received by Sponsor
on account of technical problems, human error or traffic congestion on the Internet or at
any website, or any combination thereof, including any injury or damage to Entrant's or
any other person's computer relating to or resulting from participation in this Promotion
or the downloading of any materials in this Promotion. CAUTION: ANY ATTEMPT TO
DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE
OPERATION OF THE PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAWS
AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE
RIGHT TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON(S)
RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT PERMITTED BY
LAW.
Disputes: Except where prohibited, you agree that: (i) any and all disputes, claims and
causes of action (collectively, “Disputes”) arising out of, or connected with, the Promotion
(excluding, for this purpose, any and all Disputes arising out of, or connected with, the
prizes) shall be resolved individually, without resort to any form of class action, and
exclusively by the appropriate United States District Court or the appropriate Pennsylvania
Court located in Allegheny County; (ii) any and all Disputes arising out of, or connected
with, the prizes shall be resolved individually, without resort to any form of class action,
and exclusively by the appropriate court located in Orange County, Florida; and (iii) any
and all claims, judgments and awards shall be limited to actual out-of-pocket costs
incurred, including costs associated with entering this Promotion, but excluding attorneys'
fees. EXCEPT TO THE LIMITED EXTENT THE FOLLOWING LIMITATION OF LIABILITY
IS PROHIBITED BY APPLICABLE LAW, UNDER NO CIRCUMSTANCES WILL
ENTRANT BE PERMITTED TO OBTAIN AWARDS FOR, AND ENTRANT HEREBY
WAIVES ALL RIGHTS TO CLAIM, PUNITIVE, INCIDENTAL, INDIRECT AND
CONSEQUENTIAL DAMAGES AND ANY OTHER DAMAGES, OTHER THAN ACTUAL
OUT-OF-POCKET EXPENSES (EXCLUDING ATTORNEYS’ FEES), ENTRANT WAIVES
ANY AND ALL RIGHTS TO HAVE DAMAGES MULTIPLIED OR OTHERWISE
INCREASED. All issues and questions concerning the construction, validity, interpretation
and enforceability of these Official Rules shall be governed by, and construed in
accordance with, the laws of the State of Pennsylvania without giving effect to any choice
of law or conflict of law rules (whether of the State of Pennsylvania or any other
jurisdiction), which would cause the application of the laws of any jurisdiction other than
the State of Pennsylvania.

and you hereby consent to the posting of your name if chosen as a winner.
