Health and social care strategies and plans, e.g. the Carers’ Strategy, Health Improvement Strategy for Older People and the Health Inequalities Action Plan. Performance manage delivery of Hertfordshire’s LAA HCOP theme; Develop partnership working with cross cutting themes. Produce a Health and Wellbeing Strategy and Action Plans for implementation. Outcome Two Information and advice Web based Information Health and Wellbeing pages on Hertsdirect – using NHS Choices local info Developing a database of voluntary sector organisations accessible online Written Information fiftymatters magazine – funding magazine with editorial control and development of Health and Wellbeing pages Review ACS public information and identify alternatives e.g. community television Face to Face Herts County Show – featuring health and wellbeing GP Information Service – PCT to fund pilot 2009/10 £50k secured then PBCs fund – Watford, Stevenage and Welwyn Hatfield First Contact Checklist Community Agents Community Agents – reaching out to older people living in areas of deprivation in Hertfordshire Provides face to face information and support – to be delivered in the 25 most deprived wards plus 1 ward in East Herts, 1 in St. Albans and 1 in rural North Herts. Puts people in direct contact with the agencies that are able to provide the services they need. Provides high quality information and monitors gaps service provision. Provides opportunities for people to become active members of their local communities and helps build community cohesion. Service available predominantly for people aged 50 plus but may be accessible to other disadvantaged and isolated people. It is estimated that agents will make approximately 20,000 contacts per year First Contact Signposting Scheme Multi-agency checklist enabling people over 50 and other disadvantaged and isolated people to access a range of low level ‘preventative’ services. Partner agencies e.g. Police; Fire and Rescue; Health professionals; Pension Service; voluntary organisations; Local Authorities are trained to use a 10 point checklist when visiting people at home. Completed checklists are sent to a central point, from where referrals are made to the appropriate agencies. Access to services/information, previously unknown, is made available e.g. handyman, falls prevention, carers support, winter warmth, benefits advice, smoke alarms, home security, housing options, social networks, volunteering, transport, equipment and repairs. Estimated referrals per year 18,000 First Contact Signposting and Community Agents Scheme Resources One FC coordinator per District/Borough Council area (18.5 hours per week) 28 Community Agents (10 hours per week – 10 will work 28.5 hrs in combined roles) Administration support (for both schemes) Project Manager (for both schemes) 2 x Area Managers (for both schemes) Publicity Laptops/mobile phones/travel Customer Service Centre facilitation Cost: approximately £400,000 funded through the Social Care Reform Grant for two years.
