STEP 1:
Purchase a Quantum™ Classic Black bowling ball on or before October 31, 2017 from your favorite bowling retailer. You will receive one free special edition

STEP 2:
A) Cut out ENTIRE Proof of Purchase from side of box.
Original only - no photo copies will be accepted.
B) Provide a photocopy of the original purchase receipt.
C) Enter the ball serial number on this form. Find your
serial number engraved on your bowling ball. It begins
with a letter (ex. KJE51236).
