After completing the order form, simply email it to [email protected] We welcome your questions and inquiries by phone during the form completion. However, we no longer accept fax/phone orders. Last minute changes to your order (such as quantity or substitutions) may be accepted by phone/email. The Regional Tissue Bank will assign a unique Client ID number to each facility/institution. The Client ID number will be provided to you by phone/email.
