Norfolk FA Central Player Registration Form – Guidance Notes
Please note that all Norfolk Combined Youth League players are to be registered via the
Whole Game System and not via paper registration forms.
A Parent’s Guide to Completing the Youth Registration Form
Please ensure that you complete the Player Registration Form carefully in black ink and block capitals, as
incomplete or illegible forms will be returned unprocessed and your child won’t be registered to play for their
Club.
Section One – Club / Team Details
Enter the name of the Club that you wish your child to sign for. Please also enter the name of the team that
your child will be playing for.
E.g. Enter ‘My Club United’ in the Club name and ‘blues’ within the team name section.
In addition to this, tick the age group that your child will be playing in for their Club. If you are unsure of either
the team name or age group your child will be playing for please contact your Club Official.
Section Two – Player Details
In order for the Registration Form to be accepted, you must provide the following details for your child:




Your child’s forename(s)
Your child’s surname
Their date of birth
Their gender
Section Three – Parent/ Guardian details
You must complete all the information detailed within this section:



Your forename
Your surname
Your address including postcode
Section Four – Contact details
Additionally you must provide at least one of the two requested contact details:


Email
Mobile
Section Five – League Details
In order for your child to be registered for their Club in a given League, please sign in the box next to the
league that they’ll be playing in. Please note the Norfolk Youth Combination, South Norfolk Youth League and
the North East Norfolk Youth League have now merged and are known as the Norfolk Combined Youth
Football League.
Please sign next to as many Leagues you wish your child to be registered in. You only need to complete one
Registration Form per Club, no matter how many different leagues your child wishes to sign in. So, for
example, if their Club has a Tigers team playing in the Norfolk Combined Youth Football League and a Lions

team playing in the Norfolk and Suffolk Youth League then you should sign in the boxes next to those two
Leagues.
Important points to remember





If your child wishes to sign for Club A in one league and Club B in a different league then you must
complete two separate Player Registration Forms for your child, one per Club.
This form is to be used for new registrations only. Transfers between Clubs should be notified to the
League directly and completed on the appropriate League transfer form.
Please note your child is not able to sign for more than one Club in a league. If they wish to sign for a
different Club in the same League then the transfer process must apply.
Development Football – In development football (U7-U11) you can sign for all teams within your
eligible age groups for one club with the exception of Trophy events, if the relevant league rules
permit.
Competitive Football – In competitive football (U12-U18) you can only sign for one team within your
club.
Section Six – Player Registration Agreement
Please take a moment to read the agreement on behalf of your child and then sign and date the form. The
registration form must also be signed by a club official from the Club your child is registering for.
Section Seven – Match Day Registrations
Should the League that you are registering in permit players to sign on the day of a game, then the Match day
section of the form needs to be completed.
This requires the Oppositions Official Signature and Name & Club boxes to be completed. You must also tick
the relevant League box to identify the league in which the match day registration is being completed in.
Please remember that not all Leagues allow players to sign on a match day so check the appropriate League
rules carefully.
Once you’ve completed your child’s registration form please return the form to your Club Secretary / Club
