1.
Purpose
The purpose of this document is to present the evacuation plan details for the staff of
Health Services to follow in the event that the St. Liam facility must be evacuated under
varied circumstances. These may be divided into
a. Short Duration
b. Medium Duration
c. Long Duration
And also may include
d. In Case of Fire
e. In case of Severe Weather/ Tornado
f. In case of an ND Alert
g. In case of a Shooter or Hostage Situation
1.1
ALL HAZARDS - In Preparation for any Evacuations
All employees should familiarize themselves with the location of stairs and emergency exits
within the buildings in which they work. All Staff should be trained in evacuation procedures
and in dealing with patients and other staff who may become confused or panic in an
emergency situation. All should know their duties in the event of an evacuation. Information
should be given at the time of hire and reviewed annually. (This is in addition to any required
drills or training exercises.)
At the time of an Evacuation
In the event of a situation that calls for increased staffing or an evacuation. Notify the Director
immediately. (See phone numbers below)

The Director will notify and give any information/instructions to the administrative
Team. They in turn will contact their staffs using the current UHS Phone Tree.
Those staff members who are to call others will need to relay the information
/instructions provided to them.

PLEASE NOTE! In the clinical call up, using the Phone Tree, the Assistant Director Clinical will call those in the top row and they in turn will call all those in their column. If
someone in the top row does not answer, Mary Ellen will call that group. Make a
second attempt to contact those individuals who were not initially available. If this
attempt is unsuccessful, please leave a message, text, or use any other method of
communications available to make contact.
(as in the Phone Tree) The top row will report status of cascade, including names of
personnel contacted/not contacted and availability to Jackie, Administrative Assistant,
or as per directions given when called.
While the phone Tree is being activated an email shall also be sent to all. Use current
email list for which a link in the (cell) Phone Tree and is also posted on “uhsrn” webpage
under the Phone Tree.

1.2
All staff, patients, and visitors:
In the event of an evacuation, all would evacuate to parking lot A15. The Drive Away Kit
1 and the First Aid Drive Away Kit (both at Front Desk) should be taken along by
someone from the Front Desk (M-F, 9-5). Exit out the nearest exit. See additional notes
on page 4 of this document.
The Director, an Assistant Director, or highest seniority nurse should Identify themselves to
responding emergency personnel and provide any information or assistance, as requested.
Walk to the nearest usable exit, and exit.
If the door to the stairwell is hot, or smoke or other hazardous condition is encountered or
visible, proceed to an alternate stairwell.
b) Do not use the elevators. They may lose electrical power and trap you, or may automatically
open into a fire or other hazardous situation.
c) If you have information about persons in the building who may need assistance, or about
hazardous conditions existing in the building, report it to responding emergency personnel
(for example a Police Officer, Fire Department personnel or an emergency coordinator on
arriving on the scene.

d) All persons leaving the building should close as many doors as possible and especially
hallway and stairwell doors, as they evacuate the building. (Checking that all persons have
left the facility.)
e) DO NOT RE-ENTER Facility for any reason until it is cleared to do so by the Fire Department
or word is received from ND Security or the director.
1.4
Evacuation
a) Short Term Evacuation
If the evacuation for St. Liam will be of short duration, the priority would be to provide
First Aid and Triage at a convenient location. Local Medical Facilities would be utilized
for treatment. (See current list on the Phone Tree for cell phones, or the “uhsrn” or
Business Continuity website.)
b) Medium Term Evacuation
The director may ask EOC for an alternate site. Arrangements would need to be made for supplies,
equipment, and medications (Walgreen’s) dependiong on level of care to be given

3) Long Term Evacuation
The director along with the EOC will determine if an alternate site will be used.

2. St. Liam Hall Evacuation in case of Fire
2.1




The response of the staff should be:
Call 911 or activate the fire alarm before attempting to put out a fire with a fire
extinguisher. Only attempt to extinguish the fire if it is small and you have an
appropriate fire extinguisher and are comfortable in using it.
Confine the fire or smoke by shutting doors and windows
Evacuate patients from fire area to a secure and safe area where they can easily be
removed from building.
Quickly exit the area, making sure everyone is out and close the doors as you leave
to stop the spread of fire.

Always check doors with the back of your hand before opening them – If a door is
hot, DO NOT OPEN IT as there is fire and toxic smoke on the other side. If a door is
cool, carefully open it and proceed to the closest exit, staying low to the floor.
If you cannot use the door, stuff towels and/or clothing under it to keep smoke out
and call 911 (cell phone users should use 574-631-5555) to notify NDFD of the fire
and your location.
Use the closest stairwell and NEVER USE AN ELEVATOR. If the closest stairway has
smoke in it, close the door leading to it and find another way out.
All should evacuate the building and NEVER go back inside the building to retrieve
belongings – they can be replaced but you cannot.
After exiting the building, move a safe distance (to the A15 parking lot) from the
building to avoid falling glass and other debris. Make sure that everyone is accounted
for.
The Blue First Aid Kit and Drive Away Kit #1 from room 126 should be taken
along by the Allergy nurse or if not on duty by the nurse from first floor. Exit out
the door by X-Ray. First Aid Care can be continued in A15 unless inclement
weather requires temporary indoor location. Director or designee arranges
for temporary location with the assistance of ND Security/Police or the Fire
Department.
Drive Away Kits have a copy of the current Phone Tree and further directi ons
for the continuing care of patients. (Outside pocket of Kit)
Do not reenter the facility until instructed to do so by the Fire Department

Employ additional necessary help to move patient, especially down the stairs. With due care,
stairways pose no problem in any of the above-mentioned methods; continue on down
the stairway.
Other Considerations
 Personnel needed to do the job of evacuation on scene.
 How long for additional assistance to arrive.
 Where are patients or victims to be moved (out of room, wing,
floor, building).
 Existing evacuation obstacles.
 When moving patients, move them clear of the danger area, where the fire
department or other responders may help. Do not block hall or stairways. Employ
additional help as needed to move patient.
 The outside weather at the time of evacuation – will an inside area need to
be acquired for temporary use?
 Will patients need to be transported to another facility?
 Will Patient charts be needed to be accessed?

Evacuation where persons should NOT leave the building
3.1
Instructions
In the case of evacuation where persons should NOT leave the facility such as a
Tornado, Severe Weather, or an ND Alert:
a) NEVER USE ELEVATORS - remember you may be in total darkness.
b) If in St. Liam Hall, please go to the nearest safe space.
c)
“Safe Space” As directed by Office of Risk Management and the University Architect,
we no longer have to move staff or patients to the basement areas for weather related
emergencies. Saint Liam Hall is built with poured and placed concrete that enables us to
be protected in our inner corridors or windowless rooms. If there are windows at the end
of a corridor, a “reasonable’ distance from them is suggested.
d) Remain in place until an “all clear” signal is given. ND Security will issue an “all clear”
message when it is time.
4.
4.1
Business Continuity
What is Business Continuity?
In a continuing effort to improve Notre Dame’s response and recovery following a major
campus emergency, the University has launched a business continuity planning effort. This
effort is an extension of the work previously performed on the Campus Emergency
Preparedness and Response Plan, with attention now shifting from immediate response efforts
to the continuing of business processes and operations. Business Continuity is the ability of an
organization to provide service and support for its stakeholders and to maintain its viability
before, during, and after an interruption of normal processes. The University of Notre Dame, in
its continuing pursuit to achieve excellence, has identified significant value in establishing
sound business continuity plans to support and protect its Five Major Goals.
In 2008 a Business Continuity Planning Committee was assembled to direct the planning effort.
An original 26 key departments were identified that had a key role in the recovery of critical
University processes. In the fall of 2008 the departments began their planning activities. By the
fall of 2010, the departments had compiled all of their planning, and all planning documents
were placed into a secured, central repository website called NDContinuity.com. Each
department has taken over ownership of their departmental plans so that they can
continuously updating and test them as part of their normal business activities. Additional

departments will be added to the planning process over time in an effort to maximize the
University’s resilience to unforeseen outages and interruptions.
The plans that were developed focused on three key principals, Resilience, Response, and
Recovery. A primary objective of the business continuity planning effort is to identify processes
and improvements that would minimize or eliminate the amount of time and resources
required to resume business as usual following an unexpected disruption of normal business
activities. These plans are meant to be an ever-changing, living set of documents that are
continuously maintained and updated to reflect the department’s current information,
stakeholder needs, and dependencies as their personnel, processes, and technology change
over time.
4.2
UHS Business Continuity Plan
The objective of the Health Services Business Continuity Plan is to ensure that a viable
capability exists for University Health Services to continue its essential business functions across
a wide range of potential emergencies, specifically when the primary facility is either
threatened or inaccessible. The objectives of this BCP include:
 To ensure the continuous performance of essential business functions/operations
during an emergency.
 To protect essential facilities, equipment, records, and other assets.
 To reduce or mitigate disruptions to operations.
 To reduce loss of life, minimize damage and losses.
 To identify and designate principals and support staff to be relocated.
 To facilitate decision-making for execution of the BCP and the subsequent conduct of
operations.
 To achieve a timely and orderly recovery from the emergency and resumption of full
service to all customers.
4.3
Activating an Alternate Site Overview
The following scenarios would likely require the activation of the University Health Services
Plan:
The primary facility, the University Health Services, may be closed for normal business activities as a
result of an event or credible threat of an event that would preclude access or use of the facility and the
surrounding area. This may be due to:
 a widespread utility failure
 a natural disaster
 if there could be uncertainty regarding whether additional events such as secondary failures
could occur. explosions or cascading utility outages
 significant hazardous material incident
 civil disturbance
 terrorist or military threat or attack

The following scenario would NOT require the activation of the University Health Services
plan:
"The University Health Services facility is temporarily unavailable due to a sudden emergency that
requires the evacuation of the facility, but only for a short duration that does not impact normal
operations."
4.4
Warning Conditions
Impending events such as hurricanes or winter storms may provide ample warning for
notification of staff and identification and pre-positioning of resources in preparing for possible
activation; other types of events such as earthquakes or terrorist events, may provide no
warning.
With Warning. It is expected that, in most cases, the University Health Services will receive a
warning of at least a few hours prior to an event. This will normally enable the full execution of
the BCP Plan with a complete and orderly alert, notification, and deployment of the Relocation
Team to an assembly site or an alternate facility.
Without Warning. The ability to execute the BCP Plan following an event that occurs with
little or no warning will depend on the severity of the emergency and the number of personnel
impacted. If the deployment of the Relocation Team is not feasible because of the
unavailability or loss of personnel, including the Director, temporary leadership of the
Department will be passed to the Asst. Dir., Medical Services, as identified in Section II-10 of
this document.
Non-Duty Hours. The ability to contact members of the Relocation Team at all times,
whether during work hours or non-duty hours, is critical for ensuring that the BCP can be
activated quickly if needed. Procedures must be in place that account for notifying and
mobilizing (if necessary) the Relocation Team on extremely short notice.
Duty Hours. If an event or incident occurs during work hours, which requires relocation of the
primary facility, the BCP will be activated and available members of the Relocation Team will be
deployed as directed to support operations for the duration of the emergency. The Support
Team, or those individuals who do not have assigned roles in the BCP Plan, will either be sent
home or possibly used to provide support to the Relocation Team, if additional assistance is
required.
4.5 County Multiple Casualty Incident
NOTE! If a County M ultiple Casualty Incident has been activated, especially if the incident is on
campus, the University Health Services may be called upon for assistance. In which case, it
should prepare for an influx of patients. The EMS plan in St. Joseph County will be in effect

(casualties triaged at scene and sent to an appropriate medical care site.) We could expect to be
seeing the ones with non-life threatening injuries (Most likely “Green Category” – minor injuries
or possibly some “Yellow category since we have X-Ray capability). UHS staff would use the
Medical Services incident report form. Forms should go to the Assistant Director-Medical
Services who will forward them to Risk Management.
4.6 Overview of UHS Staff Responsibilities (Quick Checklist)
The Director may activate the Business Continuity Plan (BCP) including activation of an
alternate facility.
