Office policies
1. Please arrive 15 minutes before your scheduled appointment time. This will allow us to check
your forms and ensure a full appointment for you within the time allotted. To ensure our
providers stay on schedule, if you’re more than 10 minutes late, your appointment will be
rescheduled.
2. A minimum 24 hour cancellation notice is mandatory. We understand that sometimes due to
unforeseen circumstances you may not be unable to make your appointment. Please call us as
early as possible to cancel your scheduled appointment. If you fail to notify us at least 24 hours
in advance we will charge a $35 no-show fee for an office visit or ultrasound and a $75 no-show
fee for procedure visit. When the patient fails to show up for an appointment or procedure we
have lost the opportunity to fill that time slot with another patient. The fee charged helps to
cover lost revenue as well as prepared supplies that must be discarded if you do not show up for
your appointment. Payment of these fees will need to be paid before another appointment can
be scheduled. I was under the impression we could not refuse service to some insurance
patients if they have arrangements with the “finance office.”
3. Excessive cancellations: We believe that healthcare is a partnership of care between you and
your healthcare provider and repeated cancellations without adequate notice may result in
dismissal from the practice. We only allowed 2 no-show appointments for new patients and 3
no-show appointments per calendar year for established patients. Unfortunately our practice
will not be able to schedule any other appointments if this occurs.
4. Co-payments/Co-Insurance: All copayment and insurance amounts will be due at the time of
service. We except Cash, checks and credit cards to cover these amounts. If you do not come
prepared to pay your balance, your appointment will be rescheduled.
5. Form fees: We will forward your records directly to another medical practitioner as a courtesy
at no charge. Completion of forms is not considered direct patient care. For this reason there
will be a $15 fee for each form we complete in our office for any outside organization or facility.
When forms are completed, you’re responsible for picking up the form and paying the fees.
6. Child care policy: We understand that it may be necessary from time to time to bring your
young ones with you to your office visits. We welcome families to the office visits and consider
our office a family friendly atmosphere. However, please note that our office space and
examination rooms are adequate for providing services to adult and adolescent women and that
these areas are not “child proof.” Please, monitor your child carefully and prevent them from
handling the computers, office equipment or going through the garbage. Please do not allow
them to remove items from drawers or desktops. Please do not allow them to run around the
office unsupervised. We do not provide childcare nor do we have a designated child’s area. If
your child becomes unruly or disruptive we may ask that you reschedule your appointment for a
time when you can arrange for childcare.
7. Automated calls: We sometimes will use a system that provides automated calls regarding
important notifications that may need to be sent to you. Regulations require us to get your
permission for automated calling. By signing below you agree to accept automated calls from
