Waiver of Liability and Assumption of Risk Agreement
For Catalyst Soccer Camp Participants
Risk of injury in any type of exercise or sport is always a possibility and never completely preventable.
Since there is a chance, no matter how small, that a participant could be hurt during his/her attendance at
or activity in Catalyst Soccer Training services it is mandatory for every participant (and parent/guardian if
the participant is a minor) to acknowledge and accept this agreement prior to involvement in any services
provided by Catalyst Soccer Trainers, affiliates, administrators, or staff.
In consideration of participating in all activities/services associated with Catalyst Soccer Training
– whether under the supervision of Catalyst Soccer trainers, coaches, referees, and staff or not – the
participant and his/her personal representatives, assigns, subsidiaries, heirs, next of kin, employees,
licensors, and other affiliates (further known collectively as the “Accountables”) hereby release, discharge
and covenant not to sue or hold at fault for any reason Catalyst Soccer Training, Catalyst Empire Inc. and
any of its representatives, assigns, subsidiaries, heirs, next of kin, employees, licensors, and other
affiliates (further known collectively as the “Releasees”).
The Accountables liberate the Releasees from all liability for any and all claims, demands, losses,
or damages on account of any injury, including but not limited to emotional distress, death, or damage to
property, caused or alleged to be caused in whole or in part by the negligence of the Releasees.
The Releasees may make changes to this agreement or additional terms from time to time
without prior notice or consent of the Accountables. Modifications to this agreement take effect
immediately and the Releasees will make an updated copy available to the Accountables from within, or
through, the adequate communication channel(s). The Accountables understand and agree that the use
of activities/services provided by the Releasees after the date on which this agreement or additional
terms have been revised is considered by the Releasees to be acceptance of the updated Waiver of
Liability and Assumption of Risk Agreement or other additional terms by the Accountables.
The Accountables expressly agree that the foregoing Waiver of Liability and Assumption of Risk
Agreement extends to all acts of negligence by the Releasees and is intended to be as broad and
inclusive as is permitted by the law.

Registration and Assignments
Participants are pre-assigned to groups based on the age and level indicated on their digital
registration form. If a participant requests a group change via email with the Catalyst Soccer
administrator or in-person at the camp with the lead coach there is no guarantee that the ability to switch
will be available. Participants may make a “Buddy Request” for 1 person to be paired with in their group,
but there is no guarantee that the 2 people will be grouped together if their ages and levels vary. T-shirts
are provided to participants as part of their registration fee, but these are pre-ordered based on the size
indicated on their digital registration form so there is no guarantee that another size will be available upon
camp arrival if the original does not fit as desired.

Reservation and Cancellation
It is mandatory for all participants to reserve their camp attendance online through the digital
registration system provided by Catalyst Soccer prior to acceptance in and arrival at the camp. No “dropin” participants will be allowed. Participants can be registered online 24 hours a day until the final
deadline, or until the maximum attendance capacity is reached. Participants do not have the ability to
cancel their registration online. To cancel a participant’s attendance after the digital registration form and
payment has been submitted, please email [email protected] with the participant’s name
and reason for cancelling. Please see the Payment and Refunds section below for more details.
Payment and Refunds
The full camp price must be paid online, no checks or cash, in order to be accepted into the
camp. A participant’s spot in the camp cannot be reserved with a deposit or partial/prorated payment. If
a player is absent for part of the camp, the full price must still be paid online to complete the reservation,
and no refunds for missed hours/days will be granted after camp ends. If the participant informs Catalyst
Soccer administrators prior to the first day of camp that their registration needs to be cancelled then a
refund will be provided per the following terms: minus 20% fee for 72+ hour notice, and minus 50% fee for
less than 72 hour notice. There are no refunds permitted once day 1 of the camp starts.
Proper Conduct
All participants, spectators, coaches, and trainers are expected to maintain a high level of
positivity, maturity, respect, and sportsmanship before, during, and after camps. Services are conducted
in public areas so a good attitude is important to display at all times. If a participant, spectator,
administrator, or coach’s character/behavior is deemed inappropriate or unethical to the extent that their
presence diminishes the quality of the camp experience and a tactful resolution cannot be agreed to by
the parties involved then they may be asked not to return to the camp by a Catalyst Soccer mediator.
Image Use Consent
Catalyst Soccer coaches/administrators will take pictures of soccer activity and participants
throughout the course of the camp. These images will be used for marketing purposes on our website,
brochures, flyers, posters, social media, videos, banners, and other printed/digital promotional materials.
Images will not include personal information about the participants portrayed in the picture and are never
provided to third parties for outside use. Images will be used appropriately to reflect the professional and
respectable appeal of the Catalyst Soccer brand.
TO COMPLETE THIS AGREEMENT…
The adult Accountable(s) who will be physically participating in (or are responsible for the
participation of a minor) activities/services provided by the Releasees must check the “I accept the
Liability Waiver and Service Policies agreement” box on the digital registration form. A link to read the full
PDF agreement online is provided on the camp information page and is linked in the automated
confirmation email after the registration and payment is complete.
This digital acceptance statement confirms that the Accountables have completely read this PDF
contract. The Accountables fully acknowledge, agree, and understand that substantial rights may be
given up by accepting these terms, but the digital acceptance was provided freely and voluntarily and
without oral or written inducement, assurance or guarantee. The digital acceptance is intended to be a
credible and unconditional signature to release all liability on Catalyst Soccer Training, Catalyst Empire
Inc., Catalyst Soccer Affiliate Trainers, and the other Releasees to the greatest extent allowed by law.
