To finish your household’s MaineCare Review, one or more members of your household
still need to prove Citizenship and/or Identity. Failure to provide this proof on any person could
cause them to lose their MaineCare coverage.
Please ignore the rest of this letter if you have already given us the documents.
For your convenience, and in recognition of National Health Center Week, the community
health centers listed below will have a Citizenship & Identity verification trained staff member
available on-site, Monday, August 11th to Friday, August 15th, from 9:00-11:00 AM and 1:00-4:00 PM.
They will photocopy and process your original documents (no copies please) so you don’t need to
leave your originals with them. They can also help you get documents. One person can bring the
documents for the rest of the household. Look for signs that say “Citizenship & Identity Assistance”.
Bring everything you can from the list below:





Letter, if you got one, showing your household members and what they need to complete.
MaineCare or Social Security # (card is not necessary) for one person in the household.
For All:
Original Birth Certificates or alternate documents to prove Citizenship.
For Adults: Driver’s License, State ID, or alternate document to prove Identity.
For Minors: School Picture ID, or Affidavit, to prove Identity.
Do Not Worry if you don’t have everything you need.
We will process what you have completed.
If you still need other documents, we will help you get them.
If You Can’t Come in on Any of the Days Listed above, you can still bring your
documents to a DHHS Office, during normal business hours.
