Notice of Incomplete Work Date: ________/ ________ / _________ Dear Parent: This is to inform you that your child, ________________________________________________ is up to __________ incomplete assignment(s) for this quarter of the school term. Today the following assignment was incomplete: ____________________________________________________ Detentions (until 4:00 P.M.) are served on the next school day following the issue of this notice. Consequences are issued as follows: Number of notices of incomplete assignments (1) Verbal warning- no slip is sent home (2) Lose 5min. of recess, slip must be signed and returned (3) Lose 10min. of recess, slip must be signed and returned (4) Parent/teacher meeting (5) Detention (6) Detention (7) Detention plus parent/teacher meeting - student becomes ineligible from extracurricular activities until he/she completes five consecutive school days with no incomplete work (8) Detention - student becomes ineligible from extracurricular activities until he/she completes five consecutive school days with no incomplete work (9) Detention - student becomes ineligible from extracurricular activities until he/she completes five consecutive school days with no incomplete work. (10) Detention plus meeting of student, parent, teacher, and principal Please sign this form and have it returned to school at the start of the next school day. Students who fail to return this notice will not participate in recess until the slip is returned. This definition is also meant to include lost work, and work left at home- regardless of whether or not it is complete. Extracurricular activities include but are not limited to: athletic practices and games, spelling contests, art/science fairs, forensics, and non-academic school field trips.
