at the Utah State Fair
Open to children 6-12 years old.
Every child that returns a completed form on or before the
Tuesday, July 26th deadline is a winner!
Winning is very simple:
 Open to children who are between the ages of 6-12
as of September 1, 2016. Only one entry per child.
 Children read ten books of any length and fill out
the reading record form
 Reading is verified and initialed by a teacher, parent, guardian or librarian
 Children must mail their completed forms to the
Fair. Entries must be postmarked on or before July
26, 2016. No late entries will be accepted.
 Incomplete entries will not be accepted and form
must be completed in full.
All children who return their completed forms will
receive:
 Gate entry pass to the 2016 Utah State Fair which
can be used any day of the Utah State Fair
(September 8-18)
 Ribbon bookmark from the International Association
of Fairs & Expositions
 Coupon for a free drink, redeemable at the State
Fair Store.
 Coupon from the Thomas Carnival for one free ride
redeemable at the Utah State Fair Midway.

To receive awards, read and follow the rules carefully.
1. Open to children ages 6-12 years old as of 9/1/16. One entry per
child. Multiple family entries can be mailed in one envelope.
2. Ten books (of any length) must be read by child– not teacher, parent,
guardian, librarian, etc. All ten books must be listed.
3. Books counted may be read from April 1, 2016– July 26, 2016 and
may be from any source– home, public library, school, etc.
4. Each book title must be initialed by a parent, guardian or librarian.
Incomplete entries cannot be accepted and the form must be completely filled out to receive prizes. The Utah State Fair will not share
your information with outside sources.

Awards will be mailed by on or before August 31, 2016.
All information must be legible and complete to mail awards.
Awards: gate entry pass to the Fair, this individual ticket can be
used any one day of the Fair which runs September 8-18, one ribbon bookmark, one free carnival ride (limited number available),
and one coupon for a free drink at the Fair.
The Utah State Fair is not responsible for late or misdirected entries or for incomplete or illegible entries.
