To Those Who Apply for a My Number Card
The Individual Number System is a social database allowing multiple
institutions to confirm whether existing information is for the same
person. Accordingly, this leads to the efficiency and transparency of the
social security and tax systems, achieving equality and
fairness for residents. Foreign residents with residence
records (such as mid-term residents and special
permanent residents) will be notified of their number as
well.
■The My Number Card Takes Five to Six
Months to Process
Since many people apply for a My Number Card (individual number
card), it takes about five to six months from the time of application to its
actual issue.
When preparations are ready, the Nakano City Office will send
notification in order of application. A regular mail (cannot be forwarded)
that includes an “announcement of issuance of the My Number Card”
and Issue Notification (postcard) will be sent to you. If you have filed an
application, please wait until you receive this notification.
■Procedures for Picking Up a My Number Card
1) If you receive your “announcement of issuance of the My Number
Card,” call the Nakano City My Number Call Center.
2) Make a reservation for an issuing date and time.
3) Go to the City Office in person on the reserved day, and pick up the
My Number Card.
What to Bring: Notification Card, “announcement of issuance of the
My Number Card,” Issue Notification (postcard), and some form of
personal identification.
Notes:
・You must set a PIN number when your card is issued.
・If you have a Basic Resident Registration Card, please bring that with
you as well.
・Issuing is not done at branch offices.

Check Your Health with Health Checkups
Diseases such as cancer, high blood pressure, diabetes and cerebrovascular disease are often caused by an unhealthy lifestyle. Some illnesses can
only be detected with a health examination. Please undergo a City Resident Health Checkup.
Those who receive a ticket for a health checkup listed in the chart below do not need to apply. Please undergo a checkup by February 28 of next
year. If you would like to undergo a health checkup, please file an application by February 10 of next year. The standard date of age for all checkups
is March 31 of next year. Please call for more information, such as on exemption from the personally borne expense or on application procedures.

Resident’s Taxes: An Important Funding Source for Key Services
Nakano City offers various services—such as education, welfare and disaster prevention—so that all city residents can enjoy a healthy and high
quality of life. The main financial resource for these programs is taxes, such as the resident’s tax. All residents of Nakano City must share the burden
in a fair manner.
Resident’s tax refers to the sum of special city resident’s tax and the metropolitan resident’s tax, and is levied on all those who live in the city as of
January 1 each year.
Four Payment Deadlines: The First Is June 30
Resident’s tax (special city resident’s tax and the metropolitan
resident’s tax) is paid in four installments during the year. The payment
deadline for the first term of FY2016 is June 30.
Please be sure to pay your taxes before the deadline at a financial
institution, the City Office, a branch office or convenience store. If you
pay by account transfer, please be sure that your account has enough
money before the payment date.
If You Would Like Advice concerning Payment
If you have concerns such as difficulty making payments on time,
please consult the Tax Payment Consultation Desk (City Office 3F).
Since consultations can be done over the telephone, please call as early
as possible.
The City Office has a tax payment and consultation service counter
open until 8 p.m. on Tuesdays, and from 9 a.m. to 4 p.m. on the third
Sunday of the month for those who are unable to come to the City
Office during the day on weekdays. However, since these hours may be
canceled, please check the Nakano City website and information fliers
before using the services.
Please Use Account Transfer Services
If you are prone to forgetting the deadlines or it is difficult for you to
pay at a financial institution, please take advantage of account transfer
services.
If you would like to use account transfer services, complete the
designated procedures at your local financial institution or post office.
Please bring your statement, savings account bankbook and personal seal
(if available) used for your financial account. Please note that application
forms are available not only at financial institutions and post offices in
the city, but also at branch offices and at the City Office (3F, Counter No.
7).
A Reminder Will Be Sent If You Are Late in
Making a Payment
In the case of the first term of FY2016, a reminder will be mailed in
early August to those who pay after the June 30 deadline. If you do not
pay by the deadline, you may be charged a late fee. Please make your
payment as soon as possible.
Please note that if you pay at a financial institution or post office, it
takes about two weeks until the city can confirm payment. If a reminder
comes in the mail when you have already made payment, please ignore
it.
Paying at a Convenience Store
Of the statements enclosed with your tax notification, those with the
bar code for payment at convenience stores can be used to pay at any
designated convenience store listed on the back of the statement.
Please note that in the following cases, you cannot pay at a
convenience store. Please make your payment at a financial institution,
branch office or the City Office.
• If the tax amount on a single invoice is for more than ¥300,000 (in this
case, the invoice will not have a bar code).
• If the bar code cannot be read by the machine because it is bent or dirty.
For Those Who Need Taxation (Tax Payment)
Certificates for Special City Resident’s Tax
and Metropolitan Resident’s Tax
Taxation (tax payment) certificates for FY2016 special city resident’s
tax and metropolitan resident’s tax are issued at your local branch office
or City Office (1F). The handling fee is ¥300 per copy.
If a taxpayer himself/herself applies for an issue in person, he/she
should bring some form of personal identification (such as a My Number
Card or a driver’s license); if a proxy files an application on your behalf,
the person must bring a proxy letter (signed by you, or with a seal
imprint) and documentation confirming the proxy’s identification.

Don’t Forget the National Health Insurance Premium Deadlines
for June, July and August
The Nakano City National Health Insurance premiums for a given
year are paid in ten installments from June through March of the
following year. The deadline is June 30 for the first period, August 1 for
the second period, and August 31 for the third period. Please remember
to make your payment at a financial institution, post office, the City
Office, or convenience store. If you pay by account transfer, please check
your account balance before the payment date.
As a rule, Nakano City collects premiums by account transfer; if you
are unable to complete these procedures, you must pay with an invoice.
If you have a cash card for the following financial institutions, you can
complete the procedures for account transfer easily. You can complete
application procedures at City Office or at any of the five branch offices.
Please complete the procedures as soon as possible.

Photochemical Smog Alerts
Photochemical smog is likely to develop from this time of year
through summer. Special attention must be given particularly to infants,
toddlers, the elderly, and patients with respiratory illnesses, such as
asthma.
Conditions for Occurrence and Symptoms
・It commonly occur on hazy days when the temperature is high and the
wind is mild.
・Depending on the individual, symptoms include stinging eyes and pain
in the throat.

Applications for Temporary Welfare Benefit for People Living on a Pension
(for low-income elderly persons) Are Being Accepted Until July 1
This benefit is for low-income elderly persons. A one-time benefit of
¥30,000 is provided per eligible person.
An application must be filed to receive this benefit. An application
form has been mailed on April 1 to those in Nakano City who may
be eligible. If you have received an application form and have not
yet filed your application, please complete the form with the required
information, and send it back with the enclosed envelope. Applications
that are postmarked no later than July 1 (Fri.) will be accepted.
Please beware of bank transfer frauds or personal information theft
by those who claim to be involved with the Temporary Welfare Benefit
Program.
Eligibility: Of those who were eligible for the FY2015 temporary
welfare benefit*, and those who will be sixty-five years of age or older
(born on or before April 1, 1952) during FY2016
* Those eligible for the FY2015 temporary welfare benefit refers to
those for whom both 1 and 2 below apply:
1. Those who were included in the Nakano City Basic Resident Registry
as of January 1, 2015.
2. Those who were exempt from resident tax for FY2015 (per capita
rate; this does not include those who are dependents of taxpayers of
resident tax for FY2015, nor those who receive benefits such as public
financial assistance for everyday living or a benefit for Japanese
Remaining in China After the War.)
Notes for Foreign Residents
In addition to the abovementioned Conditions 1 and 2, please note the
following points:
・Those with a short-term or invalid visa are not eligible for a benefit.
・If your visa expires during the two months required to process an
application for the benefit, your visa cannot be confirmed when
providing this benefit. Please apply for this benefit after completing
renewal procedures for your visa period of stay.

Temporary Welfare Benefit Desk using the enclosed envelope.
In the case of foreign residents, you must enclose a copy of your
Residence Card or Special Permanent Resident Certificate to confirm
your visa status of residence (the visa must be valid for two months or
more after the date of application). Please make a copy of both sides of
each document, and place them in the enclosed envelope.

Dental Checkups for 5-Year-Olds Are Being Conducted
Checkup slips have been mailed to parents of a child who will turn 5
years of age during the current fiscal year. Those who receive a checkup
slip can have their child undergo a free dental checkup individually at
a dental health institution designated by Nakano City. Please have your
child undergo a checkup within the designated period.

Preschool Children Can Use Refreshing Wading Pools for Free
Wading pools are leisure water facilities measuring about thirty centimeters deep located in parks. These can be used free of charge by preschool
children together with their parents/guardians.

Cool Evening Festival—Come Take Photos Wearing a Yukata
ANIC holds its Cool Evening Festival every year. We have planned
activities such as trying on yukata, bon-odori dancing, and playing
traditional Japanese games.
You can enjoy Japanese summer traditions, There is no need to make
reservations. Simply come to the site on the day of the event, and feel
free to join the fun.
