Accordingly, this leads to the efficiency and transparency of the social security and tax systems, achieving equality and fairness for residents. Foreign residents with residence records (such as mid-term residents and special permanent residents) will be notified of their number as well. ■The My Number Card Takes Five to Six Months to Process Since many people apply for a My Number Card (individual number card), it takes about five to six months from the time of application to its actual issue. When preparations are ready, the Nakano City Office will send notification in order of application. A regular mail (cannot be forwarded) that includes an “announcement of issuance of the My Number Card” and Issue Notification (postcard) will be sent to you. If you have filed an application, please wait until you receive this notification. ■Procedures for Picking Up a My Number Card 1) If you receive your “announcement of issuance of the My Number Card,” call the Nakano City My Number Call Center. 2) Make a reservation for an issuing date and time. 3) Go to the City Office in person on the reserved day, and pick up the My Number Card. What to Bring: Notification Card, “announcement of issuance of the My Number Card,” Issue Notification (postcard), and some form of personal identification. Some illnesses can only be detected with a health examination. Please undergo a City Resident Health Checkup. Those who receive a ticket for a health checkup listed in the chart below do not need to apply. Please undergo a checkup by February 28 of next year. If you would like to undergo a health checkup, please file an application by February 10 of next year. The standard date of age for all checkups is March 31 of next year. The main financial resource for these programs is taxes, such as the resident’s tax. All residents of Nakano City must share the burden in a fair manner. Resident’s tax refers to the sum of special city resident’s tax and the metropolitan resident’s tax, and is levied on all those who live in the city as of January 1 each year. Four Payment Deadlines: The First Is June 30 Resident’s tax (special city resident’s tax and the metropolitan resident’s tax) is paid in four installments during the year. The payment deadline for the first term of FY2016 is June 30. Please be sure to pay your taxes before the deadline at a financial institution, the City Office, a branch office or convenience store. If you pay by account transfer, please be sure that your account has enough money before the payment date. If You Would Like Advice concerning Payment If you have concerns such as difficulty making payments on time, please consult the Tax Payment Consultation Desk (City Office 3F). Since consultations can be done over the telephone, please call as early as possible. The City Office has a tax payment and consultation service counter open until 8 p.m. on Tuesdays, and from 9 a.m. to 4 p.m. on the third Sunday of the month for those who are unable to come to the City Office during the day on weekdays. However, since these hours may be canceled, please check the Nakano City website and information fliers before using the services. Please Use Account Transfer Services If you are prone to forgetting the deadlines or it is difficult for you to pay at a financial institution, please take advantage of account transfer services. If you would like to use account transfer services, complete the designated procedures at your local financial institution or post office. Please bring your statement, savings account bankbook and personal seal (if available) used for your financial account. Please note that application forms are available not only at financial institutions and post offices in the city, but also at branch offices and at the City Office (3F, Counter No. 7). A Reminder Will Be Sent If You Are Late in Making a Payment In the case of the first term of FY2016, a reminder will be mailed in early August to those who pay after the June 30 deadline. If you do not pay by the deadline, you may be charged a late fee. Please make your payment as soon as possible. Please note that if you pay at a financial institution or post office, it takes about two weeks until the city can confirm payment. If a reminder comes in the mail when you have already made payment, please ignore it. Paying at a Convenience Store Of the statements enclosed with your tax notification, those with the bar code for payment at convenience stores can be used to pay at any designated convenience store listed on the back of the statement. Please note that in the following cases, you cannot pay at a convenience store. The deadline is June 30 for the first period, August 1 for the second period, and August 31 for the third period. Please remember to make your payment at a financial institution, post office, the City Office, or convenience store. If you pay by account transfer, please check your account balance before the payment date. As a rule, Nakano City collects premiums by account transfer; if you are unable to complete these procedures, you must pay with an invoice. A one-time benefit of ¥30,000 is provided per eligible person. An application must be filed to receive this benefit. An application form has been mailed on April 1 to those in Nakano City who may be eligible. If you have received an application form and have not yet filed your application, please complete the form with the required information, and send it back with the enclosed envelope. Applications that are postmarked no later than July 1 (Fri.) will be accepted. Please beware of bank transfer frauds or personal information theft by those who claim to be involved with the Temporary Welfare Benefit Program.
