Holy Lenape Valley PLAYER Agreement This AGREEMENT is entered into by and between ___________________________________________, born on __________________ (“The Player”) and Holy Lenape Valley Ice Hockey Club (“The Club”), a high school hockey organization for the Fall 2017 hockey season, which is defined as lasting from the date of signing of this agreement until April 30, 2018 (“The Season”). It is understood that the parties intend to be legally bound by this Agreement, and that this Agreement was signed, and is enforceable, in the State of New Jersey, and that all references to the Player includes males and females. 1. The player agrees to play Holy Lenape Valley amateur ice hockey exclusively for the above mentioned Club and may not register for, be evaluated for or play for any other high school level club, that would impact the current hockey season. Except those players who may also participate with any travel league run program. The Club does recognize unique tournaments and one time opportunities that are offered, and accepts that Players may participate. 2. The Player agrees that he/she will attend all of the Club’s ice practices, games, off-ice training, instructional sessions and organized team meetings, except when prevented from doing so for educational or medical reasons, in which case, the player will request permission in advance (when reasonably possible to do so) to be absent. The club acknowledges that such permission will be granted for all reasonable educational and medical requests. The player agrees that he/she will maintain his/her academic and educational course work at the highest-level possible. The Player agrees to obey the laws of the State of New Jersey and that he/she will maintain good citizenship and behavior at all Club events, as well as in his/her everyday life, recognizing that his/her behavior is a reflection upon the Club. 3. The Player agrees not to use vulgar or profane language, racial, ethnic or gender related slurs toward teammates, coaches, officials, opponents or spectators. 4. The player acknowledges they will be dismissed from the club for use of alcohol, drugs or tobacco while involved in a club activity. 5. The Player recognizes that there is no place for violent or overly aggressive play in hockey where the obvious intent is to injure or harm another player, and agrees not to participate in such conduct. 6. The Player understand that he/she is responsible for fines or costs assessed for damage to facilities, locker rooms, etc., used in conjunction with Club’s practices, games or other team events which are caused by the above named player. 7. The Player agrees to abide by all rules and regulations of SJHSIHL, the Club, and USA Hockey, Inc. 8. The Club does not guarantee nor promise that it can improve the college or professional hockey potential of the player or guarantee equal playing time. However, the club will provide, to the best of its ability, the best environment, coaches and skill development, to assist the player in realizing his/her full hockey potential. 9. The Player and Parent will sign the USA Hockey Waiver of Liability, USA Hockey Consent to Treat forms, and USA Hockey Code of Conduct form. The player further agrees to abide by all the rules of USA Hockey’s zero tolerance policy. 10. The player recognizes that he/she is part of a team at the high school level in the State of New Jersey. As such, there is no guarantee of playing times, from player to player, and that the Coach, at their discretion, is unconditionally allowed to be prejudicial in playing times of player eg: power play, penalty kill, extra man, goaltending, or during tight//competitive games. As such, a player may sit the bench to be favored or replaced by another player in the Coach's own and exclusive discretion. 11. The player is responsible to making all required payments (registration, equipment and other club payments) in full by October 1, 2017 in order to participate in any league games played by a club team. If a player has any outstanding balance as of February 1, 2018, their name will be reported to the AAHA (Atlantic Amateur Hockey Association) and they will be played on the district’s Financial Outstanding list and the player will be prohibited from playing the remainder of the season for that team. After February 15, the Atlantic Affiliate will compile a list of all registered Affiliate players who are claimed to owe an outstanding financial obligation by their previous Atlantic Affiliate team or organization. No player whose name appears on the outstanding financial list shall be rostered with another Atlantic Affiliate team (regardless of youth or adult) until the outstanding financial obligation has been discharged or satisfied to the satisfaction of the club or permission is given by the Affiliate, respective league or club. The Player acknowledges that failure to abide by the rules set forth in this agreement constitutes a breach of this Agreement and subjects the Player and his/her Parents to disciplinary action and possible expulsion from the Club.
