Pathway for Adults – Mental Health is for people who are:




single
vulnerable
experiencing poor mental health; and
are at risk of homelessness in Somerset
Access to the service is based on mental health needs
being a primary reason for referral

Since 2008 Reconnect
Floating Support
Service has provided
housing related support
to individuals living in
their own homes with a
diagnosis of dementia
or over 65 with memory
problems
Our highly trained team
support up to 70 people
per week, working
closely with other
agencies, family and
friends
Support is tailored to the needs of the
individual using a person-centred
recovery, support and safety plan
GOAL = support people
to continue living
independently and
safely in their own
homes for as long as
possible and recover a
better quality of life

Our stepped support approach
Our service delivery focuses on support that is flexible and
can meet the changing client needs.
 High: 10+ hours a support a week. Individuals
requiring this level of support long term will require
this support to be purchased through individual
budgets
 Medium: 5 -10 hours support a week
 Low:
up to 5 hours supports a week
