Facilitator Checklist for Smart Moves Evaluation This document should provide you with information on how to collect data for the Smart Moves evaluation. We have also posted information and access to downloadable documents on a website for this evaluation at http://www.ucdenver.edu/academics/colleges/PublicHealth/departments/Comm unityBehavioralHealth/Research/Pages/Smart-Moves-.aspx. Before you collect data from youth make sure you have the following ready: ___ Chrome browser is loaded onto the computer; the online survey does not work well with the Internet Explorer browser. Download Chrome: http://support.google.com/chrome/bin/answer.py?hl=en&answer=95346. ___ URL for youth survey is loaded onto computers and it is open to the first page; https://redcap.ucdenver.edu/surveys/?s=BJoZJr. ___ Media Player audio file of script for youth survey is loaded onto computers https://www.dropbox.com/s/2f03nal45irunej/SmartMovesSurveyInstructio ns.mp3. ___ Earphones are available for youth. ___ Each youth has his/her unique ID (club member number); this must be entered onto each youth survey. You may want to make cards or labels in advance with the member’s name, age, and member number. ___ Paper surveys for backup are downloadable at the website ___ Instructions/Talking Points to read for youth before they take the survey (see below). Instructions/Talking Points for Youth Survey Youth need to enter BGC member number and club – we need your help to make sure they know and enter correctly – best to give youth card with club name and their member number Ask youth not to share club number Remind the youth to submit the survey only once online at the end of the survey This is not a test. There are no right or wrong answers. If you don’t find an answer that fits exactly, use the answer that comes closest. If you need help with a question, raise your hand and quietly ask the staff member helping you today so that you don’t disturb others around you. We have a grant to help us evaluate Smart Moves in 40 Boys and Girls clubs in Colorado. The University of Colorado Denver is helping us with this evaluation. Taking this survey before and after you take the Smart Moves program helps us know how well we are doing. We also want to know if there are any differences between members who participate in Smart Moves and those who have not. Thank you very much for taking the Smart Moves survey today! Your opinion is important to us and we appreciate your time. This survey is designed to find out about you, your opinions, and your experiences. Your answers can help the Boys & Girls Club provide more interesting and enjoyable programs for you and other club members. There should be no talking during the survey. Please keep your eyes on your own survey and do not ask questions of your neighbor. If you have a question, quietly raise your hand and I will come to you. You can answer honestly because your survey won’t have your name on it and nobody seeing it will know your name. This means your answers will never be shared with your parents, friends, or anyone in your club. Your answers will be kept confidential and used to better understand the Smart Moves program. If you take the survey on paper, we will put your survey in an envelope and send it to the University. No one at the Boys and Girls Club will look at your answers. All information in the evaluation will be reported as a group; for example by age, urban/rural clubs, boys/girls. If you are taking the survey on the computer, use your headphones. You can listen to the questions by clicking on the Media Player icon loaded onto the computer while you read along and then answer the question on the survey. If you have any questions while taking the survey, please raise you hand. Do you have any questions before we start? When to administer the Youth Survey The evaluation period is from June 1, 2013 through June 1, 2015. Please administer the survey: Before (PRE Smart Moves Course Survey) and immediately after (Post Smart Moves Course Survey) each Smart Moves course for youth in Smart Moves that begin after June 1, 2013 and that end before June 1, 2015. AND For ALL youth club members age 10-15 (whether or not they have taken Smart Moves) o In the summer (no later than July 15, 2013 and July 15, 2014) o During the school year (no later than October 31, 2013 and October 31, 2014 and April 15, 2015). We expect youth to take the survey more than once during the evaluation period. Facilitator Fidelity Report Every Facilitator should complete a fidelity assessment for each SM course. Every Facilitator must add the dates and times of the SM course in the fidelity assessment (facilitator survey) URL for the fidelity survey/report is https://redcap.ucdenver.edu/surveys/?s=tfrpbj. The facilitator must complete the survey after each Smart Moves course is completed. Website We have created a website for facilitators to access all the materials needed for this evaluation. Use it to find this document, downloadable copies of the surveys (if technology fails), answers to Frequently Asked Questions (we will create and update this section regularly based on questions we receive), and instructions on how to use the Media Player for the oral script and instructions on how to download Chrome.
