SCHEDULE:
Wednesday: 3:00 PM – 6:00 PM
Thursday: 9:00 AM – 12:00 PM, (Break from 12-2) 2:00 PM – 4:00 PM
Friday: 9:00 AM – 12:00 PM, (Break from 12-2) 2:00 PM – 4:00 PM
$125 per player - (includes T-shirt) : Check made out to HHS Volleyball

I hereby authorize all coaches of the camp to act on my behalf in case of illness or injury and to administer primary First Aid. I
hereby release the clinic staff of any liability from injury occurring before, during and after camps. All players must be covered
under their parent/ guardian’s insurance. All players are medically cleared to participate in Volleyball Activities. Parents release
liability to Huffman ISD in case of injury.
