*Registration Fee includes diamond time, umpiring fees, use of a uniform and use of team equipment and player insurance as provided by
the GHBL. For details on this insurance please contact the coach/manager. No refunds will be considered after commencement of
league schedule. No player can start team play unless the entire registration fee has been paid in full. There will be a charge of $25 for all
NSF cheques.
Privacy Statement: We collect personal information from registrants for the sole purpose of organizing baseball teams to
compete as part of our minor sports association. Information is collected and disclosed for purposes of league and
tournament registration, GHBL, COBA and Baseball Ontario registration, team selection and assignment, standard league
insurance, and complying with municipal facility usage guidelines.

 I agree that I am responsible for being present at all practices and games, and will be responsible for my child.

 I understand that registration fees will not be returned after the season starts.
 I understand that I or my child may be photographed during practice, games or tournament by the press, league or
B.M.B.A for use of reporting, or any other League or B.M.B.A use.

Registration is easy! Junior/Men - Please complete the registration form on the reverse of this page and send it with your

cheques – 1 cheque for registration and 1 cheque postdated for September 1 2017 for uniform deposit to Tim
Kingsbury (Campbellville Baseball) . The uniform deposit cheque will be returned after the uniform has been returned at the
end of the season. Please fill out one registration form for each child being registered. For second and subsequent children
please check the “second (or more) player” box. Please make all cheques payable to Tim Kingsbury – (Campbellville
Tigers Baseball) All completed registration forms, registration cheques and uniform deposit cheques to the address below.

Please note: The expectations for 2017 season is that there will be 1 team formed to play in
of the GHBL at a Tier appropriate for the Teams overall ability.:
Currently GHBL expects to have Tier 1, Tier 2, Tier 3 and Tier 4 Baseball
These teams may opt to hold try-outs and should an applicant fail to make the team they are trying out
for, their registration fee will be returned in full.

However those who make a given team, once the 1 game is played registration fees will not be

refunded. All those registered prior to March 1 2017 will be guaranteed at a minimum a try-out for a
team.

If any player does not make the team, 100% of their registration fee will be refunded and if the player is
still interested in playing, we will attempt to help the player to find another team to play on.
