Backyard Makeover Give Away Sweepstakes Rules
MainSource Bank at 2105 N SR 3 Bypass, Greensburg, IN 47240 will award (1) $10,000 backyard makeover grand prize.
The grand prize award will be drawn July 10, 2017. The grand prize will be determined from a random drawing of all
entries received during the prize period. The odds of winning the grand prize are determined by the number of entries
received prior to the time of the prize drawing. MainSource Bank reserves the right at its sole discretion to suspend,
modify or terminate this promotion without notice at any time. Return of any prize notification as undeliverable will result in
disqualification and selection of an alternate winner. All prizes will be awarded.
The grand prize winner will receive a $10,000 check via delivery by branch personnel. All prizes are non-transferable.
No credit application is necessary to participate in this promotion. A credit application will not improve chances of winning.
Void where prohibited by law, rule or regulation. You may enter this promotion by obtaining an entry at any MainSource
Bank branch. Mail completed entry form to the following address: MainSource Financial Group, Attn: Marketing, 2105 N
SR 3 Bypass, Greensburg, IN 47240. No more than one entry per household is allowed. MainSource Bank is not
responsible for any illegible, misplaced, damaged or late entries.
Sweepstake entrants must (i) be legal residents of the United States; (ii) be 18 years of age or older, and (iii)
reside in a geographic area in which MainSource Bank operates; therefore limited to, Indiana, Illinois, Kentucky
and Ohio. Employees, officers and directors of MainSource Financial Group, Inc., its affiliates, subsidiaries, cobranded partners, consultants, contractors, advertising, public relations, promotion, fulfillment and marketing
agencies, their immediate families (and those living in their same households) are not eligible to win any prizes in
the promotion.
The winners must comply with all instructions in the prize notification correspondence, including the completion of an
affidavit of eligibility and liability/publicity release, and return them within fourteen days of the date on the prize notification
correspondence as well as have met all of the eligibility requirements set forth within these Rules; otherwise prize will be
forfeited and an alternate winner will be selected. Acceptance or use of a prize constitutes permission for MainSource
Financial Group, Inc., its affiliates and subsidiaries to use recipients name and likeness for advertising and promotional
purposes without compensation, unless otherwise prohibited by law.
Liability for any applicable taxes, including all federal, state, and local taxes and/or other fees that may apply to prizes are
the sole responsibility of the winner. Any required tax reporting forms will be furnished to the Internal Revenue Service. All
expenses not specifically provided for by MainSource Bank in these Rules are the sole responsibility of the winner.
Entries that are lost, late, illegible or incomplete for any reason will not be eligible. MainSource Bank reserves the right to
modify or terminate this Sweepstakes at any time or disqualify any entry in good faith. In the event of a dispute concerning
the identity of the person submitting an entry, the entry will be deemed to be submitted by the person in whose name the
winning home is titled. For winner's list, mail a self-addressed, stamped envelope within 30 days following a prize period
to MainSource Financial Group, Inc, Attn: Marketing, P.O. Box 2000, 2105 N. State Road 3 Bypass, Greensburg, Indiana
47240 including a request for the winner’s names.
By participating in the promotion, participants agree to release, discharge and hold harmless MainSource Financial
Group, Inc., its affiliates, co-branded partners, subsidiaries, retailers, agencies and their respective officers, directors,
employees and agents, from any damages which may arise out of participation in the promotion in any way or out of the
acceptance, use, misuse or possession of a prize. This promotion is conducted in English (which will be given its
everyday ordinary meaning).

Decatur County, Indiana.
