This letter summarizes the responsibilities taken on
by parents through the sacrament of Baptism.
Dear Father,
I am requesting to have my child Baptized into the Catholic Church.
I make this request realizing that there are duties and responsibilities that
will be expected of me.
Since my child is too young to speak on his/ her own, I am the one, along
with my spouse, who must see that the Life of Faith that comes with Baptism is
kept alive, nourished and given every opportunity to grow. I realize that much
depends on my own living of the faith and my own commitment to the faith.
I have read carefully the material and, after serious consideration,
I promise to do my utmost to support and teach my child to live up to his/her
Baptismal calling in the Catholic faith.
I will do all that I can to aid my child in carrying out this Faith.
