How to submit a backflow prevention device test result
online
Before you can submit a backflow prevention
device test result online you must request
Business Access and Plumbing Test Agent
access through MyCouncil.
You can submit an online backflow prevention
device test result once your MyCouncil request is
approved.

Note: Initial backflow prevention test reports
cannot be submitted online .
Enter the Tester Name and License
Number details of the test result being
submitted.
All initial reports must be sent to
[email protected] (each test report
to be attached individually as either a PDF or
Microsoft Word document).
Associated Form 4 submissions must be lodged
to Queensland Building and Construction Code
(QBCC).
Searching for an address

Enter property details into the Address
Search. The system will provide pre-emptive
text while you are typing the address.

Select the correct property from the returned
results.
7.
If you cannot locate the property, click the
Report a Data Issue link and complete the
eRequest form for Building & Plumbing with
the relevant details.
Submitting a test result

Log in to MyCouncil at
http://mycouncil.sunshinecoast.qld.gov.au.
On the right of the Dashboard, access
MyCouncil Services through your Business
Access.
The MyCouncil Services screen displays.

Scroll to Test Reports and click Submit a
Backflow Test Report.

For devices owned by Sunshine Coast
Council, Unity Water or truck air gaps or
mobile businesses that travel between
council regions, select the correct
specifically created address from the table
below.

the property a Serial Number field will
appear allowing you to enter part of the
number to refine the search results in order
to select the correct device.
12. If you cannot locate the device or the device
details are incorrect, click the Report a Data
Issue link and complete the eRequest form
for Building & Plumbing with the relevant
details.
Submitting a test result
13. Enter the Service Date as per the report.

14. Select the applicable Outcome from the drop
down list.
If you select an incorrect property at any
time you can click the Clear Address Search
to refresh the page.
15. Tested OK – enter Notes, if applicable.
Searching for a device
10. If there is less than 25 devices located on
the property all devices will display in a list
allowing you to directly select the correct
device.

16. Fixed – enter Notes of what was the
problem and how it was resolved, this is
mandatory.
Uploading the report
20. Click the Choose File button and select
your test report document.
Note: The file format can be either a PDF or
Microsoft Word document.
21. Click the Upload Document link.
17. Replaced – enter Notes of why the device
was replaced and details of the device, this
is mandatory.
18. Removed – enter Notes of why the device
was removed, the reference number for the
form 4 submitted to QBCC and the council
reference number for approval for the
removal, this is mandatory.
22. Repeat process if a second document is
required e.g. replaced devices should
include the original device failed test report
and passed replacement device test report.
23. If the incorrect file is uploaded you can
remove it by clicking the red X, then repeat
steps 20 and 21.
Submitting the report
24. Once you are finished completing the online
form sections click the Submit button.
Note: Alternatively you can click on the
Cancel link to opt out of the report
submission.
19. Failed – enter Notes of why the device is
failing and the action to be taken including if
you need assistance from council, this is
mandatory.
25. A confirmation will pop up advising
notification has been sent to your email
address, click Close to exit this screen.

Current as at August 2015
26. The confirmation email will provide the
device’s council reference number, serial
number and property address for the test
report submitted.
27. The online form will refresh to clear all test
result details and uploaded documentation,
it will retain the Tester Information and
Backflow Device Details address and
device/s found.
Note: This allows you to continue to submit
reports for all other tested devices belonging
to the same property without searching each
time.
28. Click Clear Address Search to refresh the
page.
Note: The Tester Information will retain
until changed to another tester’s details.
