WAIVER: Every player’s parent or guardian must read and sign this waiver form. Signatures on this form signify each person has read, understands and
abides by this information. By signing this Liability and Media Release Waiver, the individual player named below wishes to participate in the event
described above and recognizes that there are risks of damage or injury arising from participation in this event or from other activities that may be
associated with participation in this event. Signature represents that the below individual has no health or physical problems that will interfere with the
participation in the tournament and she/he is responsible for their own safety. A signature releases and discharges Boys & Girls Club of the Black Hills,
Hot Springs School District, sponsors and volunteers from all actions, suits and demands in law or in equity, including but not limited to, the risk of injury
from playing in the tournament.
