KEYSTONE Questionnaire
Please take a moment to answer the following questions about your school. Your answers will make it
possible for us to complete the initial setup of KEYSTONE. We will discuss your answers with you during
our Discovery meeting.
When you have completed the form, you may print the form for your records if you desire. Once the form
is completed, save it to your hard drive (we suggest saving with a name like, “SchoolName-KSQuestionnaire”); then upload the document to Basecamp at least one month prior to scheduled
implementation date.

MARKING PERIODS
For each division:
How many marking (grading) periods do you have? What are they called (Fall, Fall Term, Winter, etc.)?
Which of these include formal reports sent to parents?
GRADES
For each division:
How do teachers report grades? (Letters? Numbers? Narrative? Criteria-based? Pass/fail? Other?)
Do teachers report effort grades? If so, how: letter, number, other?
SCHEDULES
How many terms for each division do you have (Quarter 1, Quarter 2, etc.)?
What kind of school schedule(s) do you have (block, modified block, 7-day rotating, etc.)?
Do you have defined time blocks?
GPA
For each division:
Do you calculate a GPA?
How is the GPA calculated? (Which terms? To what significant digit?)
Do you have a GPE (grade point equivalency) table?
